Abstract

Ankilozan spondilit ağırlıklı olarak omurgayı etkileyen ve ciddi fonksiyonel bozukluklara yol açabilen kronik inflamatuvar bir hastalıktır. Bu çalışmada ankilozan spondilitli hastalarda konvansiyonel egzersiz programının hastalık aktivitesi, fonksiyonel durum ve yaşam kalitesi üzerindeki etkinliğini değerlendirmek amaçlandı.Modifiye New York sınıflama kriterlerine göre ankilozan spondilit tanısı almış 50 hasta çalışmaya alındı. Hastalar 2 gruba randomize edildi ve çalışma tek kör olarak yürütüldü. Grup 1'deki hastalara bir fizyoterapist gözetiminde konvansiyonel egzersiz programı uygulatıldı. Grup 2'deki hastalara aynı egzersiz programı aynı fizyoterapist tarafından öğretilip broşür şeklinde dağıtıldı ve egzersizleri evde yapmaları istendi. Egzersiz programı 50 dakika olmak üzere günde 1 kez, haftada 5 gün, 3 hafta boyunca uygulandı. Gündüz ve gece ağrısı, ESR ve CRP düzeyleri, sabah tutukluğu, BASDAI, BASFI, BASMI, spinal mobilite ölçümleri ve Nottingham Sağlık Profili (NSP) parametreleri başlangıçta, tedavi bitiminde ve tedavi bitiminden 6 ve 12 hafta sonra değerlendirildi.Çalışmayı toplam 40 hasta (gözetim eşliğinde egzersiz grubu n=21, ev egzersiz grubu n=19) tamamladı. Tedavi bitiminde her 2 grupta birçok parametrede anlamlı iyileşmeler görüldü. 6. ve 12. hafta değerlendirmelerinde anlamlı iyileşme saptanan parametre sayısı giderek azaldı. Gruplar arası karşılaştırmada gözetim eşliğinde egzersiz grubu tedavi sonrası NSP uyku skoru ile 6. hafta servikal rotasyon (sol) değerleri bakımından ev egzersiz grubuna üstündü (p<0,05).Çalışmamızın sonuçları gözetim eşliğinde ya da ev egzersiz programı şeklinde düzenlenen konvansiyonel egzersiz programının; AS'de ağrı, hastalık aktivitesi, fonksiyonel kapasite ve yaşam kalitesi üzerinde kısa dönemde etkin olduğunu gösterdi. Bu konuda daha fazla sayıda hasta içeren ve uzun dönem takipleri olan çalışmalara ihtiyaç duyulmaktadır.
Ankylosing spondylitis is a chronic inflammatory disease that predominantly affects the spine and may cause serious functional impairment. In this study it is aimed to assess efficiency of conventional exercise program on disease activity, functional status, and quality of life in patients with ankylosing spondylitis.50 patients who had a diagnosis of ankylosing spondylitis according to the modified New York classification criteria were included in the study. The patients were randomised to two groups and the study was conducted single-blind. In group 1 the patients performed a conventional exercise program supervised by a physiotherapist. In group 2 the patients were taught the same exercise program by the same physiotherapist and given as a brochure, and the patients asked to perform these exercises at home. Exercise program was practiced five days in a week for 50 minutes, and for 3 weeks. Pain in daytime and night, ESR and CRP levels, morning stiffness, BASDAI, BASFI, BASMI, spinal mobility measurements and Nottingham Health Profile (NHP) parameters were evaluated at the beginning, after the therapy and, 6 and 12 weeks after the therapy.A total of 40 patients (supervised exercise group n=21, home-based exercise group n=19) completed the study. After the therapy both groups showed significant improvements on many parameters. Number of these parameters which showed significant improvements was gradually decreased in 6th and 12th weeks. Intergroup comparisions NHP sleep subscore after the therapy, and servical rotation (left) in 6th week were superior in supervised exercise group than home-based exercise group (p<0,05).Results of our study showed that conventional exercises which is designed as a supervised exercise or home-based exercise program had short-term effects on pain, disease activity, functional capacity and quality of life in AS. Relevant to the issue there is a need for more studies those are including more patients and have long-term follow-up.
