Abstract

% 50 oksijen yüzdesi ile Halothane ve Methoxyflurane, % 70 oksijen yüzdesiyle Methoxyflurane uyguladığımız üç grup ve toplam 144 hastada ameliyatın belirli sürelerde alınan arteriyel kandan kan gazları bulgularını karşılaştırdık. Bulduğumuz sonuçlara göre; her iki akciğeri sağlam veya tek akciğerinde lezyon olan hastalarda, sırtüstü ve yan konumda yapılan ameliyatlarda % 50 oksijen yüzdesi yeterli olabilmektedir. Oysa, göğüsün açıldığı ve akciğerin kollabe edildiği ameliyatlarda ise hipoksemi için sınır kabul edilen 70 mm. Hg. in üstünde po2 sağlanması için % 70 oranında oksijen zorunludur. Her iki akciğerinde parenkim bozuklukları olan Mitral Stenozlu hastalarda % 70 oksijen yüzdesi ile bütün konumlarda normal po2 elde edilebilmektedir. Ayrıca her iki oksijen yüzdesi ile de pco2 in yüksek bulunması akciğer anestezisinin başka bir sorunudur.
Halothane and Methoxyflurane with 50 percent oxygen and Methoxyflurane 70 percent oxyfen are used in 144 patients who are in three groups. We compared the blood gas findings from arteriel blood withdrawn periodically. According to our results, in the patients who have not lesion or who have lesion in one lung. 50 percent Oxygen may be sufficient in supine or lateral position during operation. Yet, in thoracotomy and one - lung anesthesia 70 percent oxygen should be used for p02 above 70 mm. Hg. In mitral stenosis patients who have parenchimal and vascular bed destruction in both lungs normal p02 value may be obtained with 70 percent oxygen in all position during closed valvulotomy operations. In addition to these , high pCO2 findings in all position are another problem of lung anesthesia .
