Abstract

Normalde hemostazda görevli olan trombositlerin, son yıllarda, atherogenezis'de saptanan etkileri nedeniyle önemleri giderek artmaktadır. Özellikle endotel hücre (EH)'lerinde meydana gelen zedelenmeyi takiben trombositler ateroskleroz (As) olayna katılırlar. Başlangıçta EH'lerinden savunma amacıyla salgılanan PAF (trombosit aktive edici faktör), trombositlerin aktivasyonunda önemli rol oynar. PAF'la aynı zamanda EH'den salgılanan PGl2 (prostasiklin) ise vazodilatasyon yapar ve uyarılmış trombositlerin damar endotelinden uzaklaştırılmasını sağlar. Gene, EH yüzeyinde bulunan heparin, antifrombini aktive ederek, antikoagülan etki gösterir. Trombosit ve EH'Ieri arasındaki bu ilişkiler ve trombositlerden salgılanan kemotaktik ve mitojenik maddeler, özellikle de PDGF (trombositten çıkan gelişim faktörü), bir yerde olayın geleceğini belirler. Sonuçta; zedelenme derecesi, trombositlerin aletivasyon miktarı ve kişinin yatkınlığı ile orantılı olarak proliferatif atherosklerotik lezyonlar gelişir.
Normally platelets are hemostatic cells, but in recent years they are gaining importance for their roles in atherogenesis. In atherosclerosis (As), they play a role especially in the period following endothelial cell (EC) injury. Initially, from the EC's with the aim of defence PAF (platelet activating factor) is secreted. This substance activates platelets. PGI2 (prostacyclin) is also one of the substances secreted from the EC. It is secreted simultaneously with PAF and makes vasodilatation and removes away the activated platelets from the endothelial surface. In addition to PG heparin, which is found on the EC membrane surface, activating antithrombin shows anticoagulant effect. The relations between the platelets and the ECs and the chemotactic and mitogenic substances, especially PDGF (platelet-derived growth factor), released from the activated platelets determine tlıe progress of the case. In conclusion, we can say that proportionality with the injury degree and the ambit of the activated platelets and the predisposition of the individual, proliferative atlıerosclerotic lesions develop.
