İntraserebral hematomların lokalizasyonu ve büyüklüğü ile hastanın şuur durumunun prognoz tayinindeki rolü

Abstract

Bu çalışmaya U. Ü. Tıp Fakültesi Nöroloji Kliniğine tetkik ve tedavi amacı ile yatırılan 123 lnt serebral Hematomlu (İSH) olgu alındı. İSH ların Kranial CT ile anatomik lokalizasyonlar yapıldı ve hematomların büyüklüğü ölçüldü. Ayrıca olguların şuur durumları Glasgow Koma Skalasına göre değerlendirildi ve bu parametrelerin prognoz üzerine olan etkileri gözden geçirildi. Çalışmada Putaminal 34 (% 27.6), Talamik 46 (% 37.3), lobar beyaz cevher 28 (% 22.7), beyin sapı 12 (% 9.7),serebellar 3 (% 2.4) şeklinde anatomik lokalizasyon mevcut idi. 20 mm. den küçük hematomu olan 19 olgunun 4'ü (% 21), 20-45 mm. arası hematomu olan 60 olgunun 30'u (% 50), 45 mm. den büyük hematomu olan 44 olgunun 36'sı (% 81.8) öldü. Hastalardaki ölüm oranı putaminal% 44, talamik% 52, Lobar beyaz cevher % 82, beyin sapı % 58 ve serebellar hemorajilerde % 33 olarak bulundu. Ayrıca şuuru açık olan 27 olgunun 4'ü (% 14.8), somnolans halindeki 29 olgunun 12'si (% 41.3), stupor halindeki 23 olgunun 16'sı (% 69.5), prekoma halindeki 16 olgunun 14'ü (% 87.5) ve koma halindeki 28 olgunun 24'ü (% 15.7) öldü.
This study consists of 123 patients with Intracerebral hematomas who were hospitalized and treated in the Neurology department of the Medical Faculty of Uludag University. The anatomical localizations and the dimensions of the Intracerebral Hematomas were determined by cranial CT and the patient's consciousness state is determined ( according to the Glasgow-Coma Scale ). Later the effect of these parameters on the prognosis is discussed. The anatomical localization of hematomas were; putaminal 34 (27.6 % ), thalamic 46 (37.3 %), lobar white substance 28 (22.7 %), brain stern 12 (9.7 %) and cerebellar 3 (2.4 %). 4 (21 %) of 19 patients whose hematomas were smaller than 20 mm. 30 (50 %) of 60 patients whose hematomas were 20-45 mm. and 36 (81.8 % ) of 44 patients whose hematomas were larger than 45 mm. died. The mortality rate was 44 % in putaminal, 52 % in thalamic, 82 % in white lobar substance and 58 % in brain stern and 33 % in cerebellar hematoma. Also the mortality was 4 (14.8 %) of 27 patients who are alert, 12 (41.3 %) of 29 patients who are in somonolance, 16 (69.5 %) of 23 patients who are in stupor, 14 (87.5) of 16 patients who are in precoma and 24 (85.7 %) of 28 patients who are in coma.
