Abstract
R.T.20 yaşında bir kadın hasta, diyabetik koma ve prerenal akut böbrek yetmezliği tanısıyla hastaneye yatırıldı. Yatışından dört gün sonra burun üzerinde nekrotik ve gangrenöz bir lezyon ortaya çıktı ve daha sonra süratle ilerleyerek bilateral oftalmos, sol fasial paralizi, sağ abdusens paralizisi, sağ kısmi oftalmopleji, sol tam oftalmopleji, bilateral optik atrofi gelişti. Klinik olarak cerebral mucormycosis tanısı kondu ve tanı burun ifrazından yapılan kültürle ve burundaki nekrotik dokudan alınan biyopsi materyalinin histopatolojik incelenmesi ile doğrulandı. Diyabetin süratle regülasyonu, akut böbrek yetmezliğinin periton diyalizi ile düzeltilmesi ve oral mikostatin tedavisi ile hastada sekelli bir iyileşme elde edildi.
R.T., a ferma le patient, 20 years old was admitted to the hospital with diabetic coma and prerenal acute renal insufficiency. Four days after her admission, a necrotic and gangrenous lesion was noted in her nose, then, bilateral ophthalmic ptosis, left facial paralysis, right abducens paralysis, right incomplete ophthalmoplegia, left complete ophthalmoplegia and bilateral optic atrophy developed. The clinical diagnosis of rhinocerebral mucormycosis was verified with culture of the exudate from the nose and histopathologically from nasal necrotic tissue. Recovery is obtained with regulation of the diabetes and improvement of the renal insufficency by peritoneal dialysis and therapy with mycostatin.
