Abstract

Çağdaş kemoterapi protokolleri ve destekleyici tedaviler ile çocukluk çağı ALL'sinde sağkalım oranı %80'lere ulaşmıştır. Ancak; kemoterapi ile ilişkili akut yan etkiler, tedavi sırasında morbidite ve mortaliteye neden olan ciddi sorunlara neden olabilmektedir. Kemoterapiye bağlı geç dönem etkilerle ilgili çok sayıda çalışma varken, akut toksisite ile ilgili çalışmalar sınırlıdır.Bu çalışmada, BFM protokolü ile tedavi edilen ALL'li çocuklarda kemoterapiye bağlı gelişen akut yan etkiler değerlendirildi.BFM kriterlerine göre düzenlenen akut toksisite formu ile, 1993 ve 2009 yılları arasında tanı alan 210 çocuğun hastane kayıtları geriye dönük olarak her bir kemoterapi fazı için ayrı olarak değerlendirildi.Yüz seksen yedi çocuğun verileri değerlendirildi. Yaş ortalaması ve erkek/ kız oranı sırayla 62 ± 51.7 ay ve 1.7 idi. Ortanca takip süresi 61.3 ± 43.4 aydı. Olgular; standart (%19.8), orta (%61.5) ve yüksek (%18.7) olarak 3 risk grubuna ayrıldı. Kemik iliği toksisitesi; her risk grubunda indüksiyon fazında sık görülmekle birlikte, yüksek risk blok tedavileri sırasında anlamlı şekilde daha sık saptanmıştır. Kusma, stomatit, ishal ve karaciğer transaminaz yüksekliği, yüksek risk bloklarında anlamlı şekilde daha sık bulunmuştur. Olguların %31'inde (n:58) L-aspariganaza bağlı ürtiker ve/veya anafilaktik reaksiyon gelişmiştir. İndüksiyon fazında, 187 olgunun 7'sinde (%3.7) tümör lizis sendromuna bağlı geçici akut böbrek yetmezliği gelişti. Kardiyak disfonksiyon ve hipertansiyon sırayla %6.9 (n:13) ve %9.6 (n:18) olarak tespit edildi. Konvülziyon, tromboz, kanama, enfarkt ve PRES'i içeren SSS toksisitesi 20 (%10.6) olguda gözlendi. Olgu başına febril nötropeni atak sayısı 4.8 idi. Bu atakların %41.4'ünde (n:371) odak saptanırken, %23.7'sinde etken izole edildi. Evre-3 ve 4 enfeksiyon sıklığı, YRG grubunda anlamlı şekilde daha fazlaydı. Genel ve olaysız sağkalım oranları sırayla %86.1 ve %82.9 olarak bulundu. Yirmi beş (%13.3) olgu, relaps/ dirençli hastalık (n:13, %6.9) ya da tedaviyle ilişkili toksisite (n:12, %6.4) nedeniyle kaybedildi. Nötropenik sepsis (n:9, %4.8), tedavi sırasında en önemli ölüm nedeniydi. İki olgu da idame tedavisi sırasında, aşı ile önlenebilir hastalıklar nedeniyle kaybedildi.İndüksiyon ve yüksek risk blok tedavileri sırasında evre-3 ve 4 toksisite daha sık olarak gelişmektedir. Remisyondaki olgularda en önemli mortalite nedeni olarak enfeksiyonlar yer almaktadır. Bu nedenle; uygulayıcılar kemoterapiye bağlı gelişebilecek akut yan etkiler konusunda dikkatli olmalı ve erken dönemde uygun destek tedavi ile bu yan etkiler en aza indirilmelidir.
The survival rate in childhood ALL has reached to 80% with the contemporary chemotherapy protocols and increased supportive care. However, chemotherapy related acute side effects may stil cause serious problems leading to morbidity and mortality during treatment. Although the reports about the late effects among the survivors are frequent, the data related to the acute adverse effects are scarce.This study evaluated the acute side effects of chemotherapy in children with ALL treated with BFM based protocols.The acute toxicity according to the BFM criteria for each chemotherapy phase in 210 children treated as ALL between the years 1993 and 2009 were, retrospectively, evaluated from their hospital records.The data of 187 children were evaluable. The mean age and male to female ratio were 62 ± 51.7 months and 1.7, respectively. The median follow-up period was 61.3 ± 43.4 months. Children were stratified into 3 risk groups as standart (19.8%), median (61.5%) and high (18.7%). Bone marrow toxicity was more prominent during induction phase in each risk group, however, it was significantly depressed during high risk blocks. Vomiting, stomatitis, diarrhea were significantly occurred in high risk blocks along with elevated liver transaminases. Allergic drug reactions due to L-asparaginase including urticaria and/or anaphylaxis occured in 31% (n:58) of children. Seven out of 187 children (3.7%) developed tumor lysis syndrome with reversible acute renal insufficiency during induction phase. Cardiac dysfunction and hypertension were determined in 6.9% (n:13) and 9.6% (n:18) of the children, respectively. CNS toxicity including, seizures, thrombosis, hemorrhage, infarct and PRES was observed in 20 (10.6%) children. The number of febrile episodes per patient was 4.8. The primary site of infection could be determined in %41.4 (n:371) of these episodes. Causative microbiologic agent could be demonstrated in 23.7%. The frequency of grade-3 and 4 infections were significantly more in HRG. Overall and event free survival rates were found 86.1% and 82.9%, respectively. Twenty-five patients (13.3%) died of either treatment related toxicity (n:12, 6.4%) or relapsed/ refractory disease (n:13, 6.9%). Neutropenic sepsis (n:9, 4.8%) was the major cause of death during treatment. Two cases was also lost due to preventable diseases with vaccination during maintenance therapy.Treatment-related grade-3 and 4 toxicity was more prominent during the induction phase and HRG blocks. Infections are the most important cause of mortality in remission. For this reason; practitioners should be careful about the acute side effects of chemotherapy and in the early period with appropriate support treatment, these side effects minimized.
