Dental girişim uygulanmış hastalardaki oral mukoza lezyonlarında kontakt alerjinin rolü

Abstract

Kimyasal maddeler, oral mukozada irritatif ve alerjik reaksiyonlara neden olarak farklı belirti ve bulgularla seyreden inflamatuvar hastalıklara yol açarlar. Bu çalışmada ağız içine dental girişim uygulanmış olan kontakt stomatit, oral liken planus ve rekürren aftöz stomatitli hastalarda, standart seri ve dental seri ile yama testi uygulanarak; saptanan alerjenlerin türü, alerjen-lezyon ilişkisi, alerjen- dental işlem ilişkisi araştırılmaktadır. Böylece oral mukozada görülen alerjik reaksiyonlarda etyolojik faktörler tesbit edilerek tanı ve tedavide yeni yaklaşımlara ışık tutulması amaçlanmıştır.Çalışmaya kontakt stomatit, oral liken planus ve rekürren aftöz stomatitli, her gruptan 20'şer hasta olmak üzere, toplam 60 hasta, kontrol grubu olarak; dental girişim uygulanmış 15 sağlıklı birey ve dental girişim uygulanmamış 15 sağlıklı birey alındı. Hasta ve kontrol gruplarına 30 maddeden oluşan dental seri alerjenleri ve standart seri içersindeki 20 alerjen ile yama testi uygulandı. Sonuçlar Uluslararası Kontakt Dermatit Araştırma Grubu kriterlerine göre 48. ve 96. saatlerde değerlendirildi.Gruplar arasında yaş, cinsiyetlere göre dental seri ve standart seriyle yama testi pozitifliği yönünden anlamlı fark yoktu (p>0.05). Hasta grubunda yer alan 60 kişiden 44'ünde dental seri, 11'inde standart seri alerjenlerine karşı pozitiflik saptanırken, kontrol grubunda 8 kişide dental seriye, 1 kişide de standart seriye karşı pozitiflik kayıt edildi. Hasta grubu ile kontrol grubu dental ve standart seri ile yama testi pozitifliği açısından karşılaştırıldığında anlamlı bir farklılık saptandı (p<0.001). Goldsodiumthiosulphate (%30), nickel sulfate (%25), cobalt chloride (%23.3), methylhydroqinone (%16.6) ve mercury (%15) dental seri yama testinde pozitiflik saptanan en sık alerjenler idi. Standart seri yama testinde ise en sık formaldehyde water (% 5), fragrance mix (% 5) ile pozitif reaksiyon saptandı.Dental işlem uygulama zamanı ile yama testi pozitifliği arasında ilişki yoktu. Dental girişimi dolgu olanlarda en sık rastlanan alerjen nickel sulfate; protez olanlarda goldsodiumthiosulfate; köprü-kaplama olanlarda ise cobalt chloride olarak tesbit edildi. Dolgu ve protezli hastalarda dental seri yama testi pozitifliğinde kontrol grubuna göre istatiksel olarak anlamlı bir farklılık saptanırken (p ? 0.05), köprü-kaplama işlemi olan, her iki ve üç işlem uygulanmış hastalar arasında anlamlı bir fark yoktu (p>0.05).Bu çalışmada dental işlem uygulamalarından sonra mukozanın alerjenlerle teması sonucu kontakt alerjik reaksiyonların %73.3 oranında geliştiği gösterilmiştir. Metallere, tatlandırıcı ve koruyuculara karşı gelişen yama testi pozitiflikleri, kontakt alerjinin oral mukoza hastalıklarının oluşumunda önemli bir yeri olduğunu düşündürmektedir. Bu sonuç, uygun alerjenlerle yapılacak daha fazla ve daha geniş olgu serili yama testi çalışmaları ile desteklenmelidir.
Chemical substances lead to different types of inflammatory diseases by causing irritative and allergic reactions. In this study, patch tests with standart and dental series were applied to contact stomatitis, oral lichen planus and recurrent aphtous stomatitis patients who had dental restorations or orthodontic devices to search for the types of allergens detected, the relationship between the allergen and the lesion, and the relationship between the allergen and the type of dental procedure. By this way, new diagnostical and therapeutic approcahes for detecting etiological factors causing allergic reactions in the oral cavity may arise.Twenty patients for each group of oral diseases (contact stomatitis, oral lichen planus and recurrent aphtous stomatitis), 15 healthy people without any dental restorations and 15 healthy people with a history of dental restorations or orthodontic device application were incleded in this study. Allergen patch testing with 30 allergens in dental series and 20 allergens in standart series was performed to patient and control groups. Readings were obtained at 48 and 96 hours and patch test reactions were evaluated according to International Contact Dermatitis Research Group criteria.There was no statistically significant difference between groups according to age and gender at patch testing with standart and dental series (p>0.05). In the patient group, 44 of 60 patients showed at least one positive patch test result at dental series and 11 of 60 showed at least one positive positive patch test result at standart series. In the control group, 8 people were noted to be allergic to dental series and 1 to standart series. Statistically significant difference were found between patient and control groups according to results of standart and dental seires patch testing (p<0.001). Goldsodiumthiosulfate (30%), nickel sulphate (25%), cobalt chloride (23.3%), methylhydroqinone (16.6%) and mercury (15%) were the most common allergens detected in the dental series patch testing. Most positive reactions were detected with formaldehyde water (5%) and fragrance mix (5%) in standart series patch testing.No relationship was found between the date of dental procedure and patch test positivity. Nickel sulphate was found to be the most common allergen in patients with dental fillings, whereas goldsodiumthiosulfate was the most common allergen in patients with dental prothesis and cobalt chloride with bridging patients. Dental series patch test results in dental filling and prothesis patients showed statistically significant positivity against control group. However, bridging patients group alone or in combination with other dental application groups showed no statistically significant importance according to patch test results.Contact allergic reactions secondary to mucosal allergen exposure caused by dental procedures were seen in 73.3% of patients in this study. The patch test positivity against metals, flavorings and preservatives showed that contact allergies play an important role in the etiology of oral mucosal diseases. This result should be supported by wide patient series patch test studies with appropriate allergens.
