Abstract

Bitkisel tıp ürünleri tüm dünyada her geçen gün daha fazla oranda kullanılmakta, ancak olası yararlı ve zararlı etkileri tam olarak bilinmemektedir. Aloe vera kullanan hastalarda kolonik anastomoz yapıldığı takdirde ne gibi sonuçlarla karşıtaşılacağını incelemek üzere deneysel çalışma yapıldı. 96 adet rat 3 gruba ayrıldı ve inen kolon transeksiyonu sonrası anastomoz oluşturuldu. Gruplardan birine yalnızca preoperatif, diğerine preoperatif ve postoperatif dönemde 1 ay süreyle aloe vera gel 1,6 mg/kg dozunda orogastrik feeding kateter ile verildi. Üçüncü grup kontrol grubu olarak standart diet ile beslendi. 16 adet rat sıfırıncı günü temsil etmek üzere Sham-aloe ve Sham-kontrol olmak üzere 2 ayrı gruba daha ayrıldı, anastomoz yapılmaksızın kolon patlama basıncı ve hidroksiprolin düzeyleri ölçüldü. Anastomoz yapılan denekler 3,7, 14 ve 21. günlerde sakrifiye edildi. Anastomozlar incelendi, patlama basınçları ölçüldü. Yara iyileşmesindeki özellikleri histopatolojik yöntemle (parçalı lökosit, yeni kapiller damar oluşumu, fibroblast proliferasyonu ve kollajen miktarları) ve biokimyasal yöntemle (hidroksiprolin düzeyi) incelendi. Sıfırıncı günde kontrol grubu hidroksiprolin içeriği daha az olmakla beraber (p>0.05) kolon patlama basıncı aloe vera grubundan anlamlı olarak yüksekti (p=0.0002). Aloe vera verilen her iki grupta anastomoz sonrası 3. günde makroskopik kaçaklar saptandı. Aynı dönem patlama basınçları da kontrol grubuna göre anlamlı olarak düşüktü (p=0.007 ve p=0.003). Bu dönem içerisindeki hidroksiprolin içeriği değerlendirildiğinde aloe vera vermeye devam edilen grupta, kontrol grubuna göre anlamlı ölçüde (p=0.05) düşüklük saptanırken, aloe vera kullanımı kesilen ikinci grupta belirgin düşüklük devam ediyordu (p=0.054). Doku hidroksiprolin düzeyi ve patlama basıncı açısından 3. günde gözlenen bu fark 7, 14 ve 21 . günlerde izlenmedi. Dokuların histopatoloijk incelenmesinde ise skorlama sonuçları açısından gruplar arasında anlamlı fark saptanamadı. Bu çalışmada aloe veranın dokularda kollajen içeriğini arttırdığı saptanmış olmasına rağmen kollajen içeriği travma sonrası erken dönemde ciddi düzeyde azalıp kaçaklara sebebiyet vermektedir.
The worldwide use of herbal medicine products is increasing everyday without knowing possible beneficial and side effects. We examined effects of Aloe vera on colonic anastomosis in rats demonstrating patients using Aloe vera. 96 rats were randomized in three groups. An anastomosis was performed after transection of descending colon in all rats. Rats in the first group received daily Aloe vera gel (1 ,6 mg/kg) preoperatively for one month and continued to recieve postoperatively via orogastric feeding catheter. Same procedure was followed for the rats in the second group but discontinued to feed with Aloe vera postoperatively. Third group served as a control group and received only standard rat chow. 16 rats were randomized in two additional groups; Sham Aloe and Sham Control to represent "Day zero". In sham groups, bursting pressures and hydroxyproline levels of the descending colon were measured without performing an anastomosis. Rats having anastomosis were sacrificed on postoperative 3rd, 7th, 14th and 21st days. Anastomoses were investigated macroscopically and bursting pressures were measured. The features of the healing process were studied histopathologically (inflammatory cell infitration, neovascularization, fibroblast proliferation, collagen deposition) and biochemically (hydroxyproline levels). Bursting pressures of the Sham Controls were significantly higher than the Sham Aloes (p=0.0002) while hydroxyproline levels of the Sham Controls were lower (p>O.OS) controversially. At macroscopic examination of the anastomoses, leakages were noticed on postoperative day 3 in both groups · received Aloe vera. The bursting pressures of Aloe vera received groups were significantly lower than control group (p=0.007 and p=0.003). The hydroxyproline levels of the first group, received Aloe vera preoperatively and postoperatively, were sign ifıcantly lower than control group (p=0.05) whereas hydroxyproline levels of the second group were almost significantly law compared to the control group (p=0.054). On postoperative 7th, 14th and 21st days, bursting pressures and hydroxyproline levels were similar among all groups. Histopathological examination revealed no signifıcant differences between groups. The results of this study suggest that Aloe vera increases colonic collagen content, but somehow decreases it significantly after trauma, resulting in an increased risk of anastomotic leakage during early postoperative period.
