Abstract

Gastroözefageal reflü, özofagusun en sık görülen hastalığı ve çocukluk çağı hastalıkları içinde de en sık görülen hastalıklardan biridir. Gastroözofageal reflü gastrik içeriğin istemsiz olarak özofagusa hareketidir. Sağlıklı süt çocuklarında, çocuklarda ve erişkinlerde gün boyunca fizyolojik olarak oluşabilmekte, genellikle de kısa süreli ve asemptomatiktir. Gastrik içeriğin özofagusa kaçışı çocukta semptoma yol açıyor, yaşam kalitesini etkiliyor, patolojik sekel bırakıyorsa gastroözofageal reflü hastalığı olarak tanımlanmaktadır. Günümüzde birçok kronik hastalıkta yasam kalitesinin önemi üzerinde durulmaktadır. Genel olarak “kalite” iyiliğin bir derecesidir. Yasam kalitesi, kişisel sağlık durumundan öte, kişisel iyilik halini de içine alan daha geniş bir kavramdır. Bu çalışmada gastroözefageal reflü tanısı olan çocukların yaşam kalitelerini belirlemek, sağlam çocuklar ile karşılaştırmak ve yaşam kalitelerine etki eden faktörleri değerlendirmek amaçlanmıştır. Gastroözefageal reflü hastalığı olan 50 hasta ve ebeveyninin, kontrol grubu olarak 50 sağlam çocuk ve ebeveyninin sağlıkla ilişkili yaşam kalitesi değerlendirilmiştir. Hasta, kontrol grubu ve ebeveynlerine yaşam kalitesi ölçek formu doldurulması istenmiştir. Çalışma anket şeklinde yapılıp yüz yüze görüşme tekniği kullanılarak araştırmacı tarafından yapılmıştır. Hastaların 13’ü (%26) erkek, 37’si (%74) kız idi. Yaş ortalaması 12,44±3,137 idi. Kontrol grubun yaş ortalaması 12,6± 2,857 idi. Ortalama yaşam kalitesi ölçek toplam puanı hastalarda ve ebeveynlerinde kontrol grubundaki sağlam çocuk ve ebeveynlerine göre anlamlı olarak düşük saptandı (p<0,01). Bu sonuçlar literatürde yapılmış çalışmalara göre uyumlu bulunmuştur. Sonuç olarak; gastroözefageal reflü hastalığı tedavisinin uzun sürmesi, tedavi sırasında sıkı diyet uygulanması nedeni ile çocuğun günlük yaşamını aile yaşantısı bozulabilmekte ve yaşam kalitesi olumsuz etkilenmektedir. Yasam kalitesi ölçümleri, hastanın hastalığa olan tepkisini daha iyi anlamanın yanı sıra hastanın tedaviye uyumunu artırarak, destekleyici bir yaklaşım sağlamaktadır.
Gastroesophageal reflux disease (GERD) is the most frequent disease of oesophagus, and also one of the most frequently seen diseases among childhood diseases. Gastroesophageal reflux is defined as regurgitation of gastric contents into oesophagus involuntarily. In some cases, it can occur physiologically in healthy toddlers, children and adults throughout the day; it is usually short-term and asymptomatic. The condition is defined as gastrooesophageal reflux disease when the regurgitation of gastric contents into oesophagus leads to symptoms, affects the quality of life, and causes pathologic sequels in children. Currently, the importance of the quality of life is emphasized in many chronic diseases. In general, “quality” is a degree of well-being. Quality of life is a more comprehensive concept, which includes also individual well-being beyond individual health status. In this study, we aimed to identify the quality of life of children with a gastro-oesophageal reflux disease diagnosis, to compare them with healthy children, and to assess factors affect their quality of life. Health-related quality-of-life of 50 GERD patients and their parents, additionally health-related quality-of-life of 50 healthy children and their parents as controls were assessed. The patients, controls and parents were asked to complete the quality of life questionnaire form. The study was conducted as survey, and the researcher administered the questionnaire in a face-to-face manner. 13 patients (26%) were male, while 37 of them (74%) were female. The mean age of the patients was 12,44 ± 3,137 years; while it was 12,6 ± 2,857 years in the control group. The mean score of quality of life questionnaire was significantly lower in patients and in their parents, compared with healthy children and their parents in the control group (p<0,01). These results were consistent with the results of other studies in the literature. Consequently, due to long duration of treatment, and implementation of a strict diet during treatment, gastro-oesophageal reflux disease may impair daily life and domesticity of child, and affects quality of life negatively. Quality of life measurements provides a supportive approach, with increasing patient’s adherence of therapy, as well as allowing us to understand the reaction of the patient against disease better.
