Abstract

Bu çalışmada Miyofasyal ağrı sendromu (MAS) tedavisinde transkutanöz elektriksel nöral stimülasyon (TENS) ve kuru iğneleme tedavilerinin etkinliğinin karşılaştırılması amaçlandı. Çalışmaya 18-60 yaş arası en az 3 aydır üst trapezius kasında MAS olan 50 hasta alındı. Hastalar numaralandırılmış zarf çekim usulü ile randomize edilerek, 25'er kişilik 2 gruba ayrıldı. 1. gruba haftada 5 gün, 2 hafta konvansiyonel TENS (50-100 Hz, 100 μsn) uygulandı. 2. gruba unilateral 2 toplamda 4 tetik noktaya 1 hafta arayla 2 defa kuru iğneleme enjeksiyonu uygulandı. Her iki gruba da ev programı olarak üst trapezius kasına yönelik germe egzersizi eğitimi verildi. Hastalar Vizüel Analog Skala, algometri cihazı ile ağrı basınç eşiği ölçümü, palpabl kas spazm derecesi, Nothingham Sağlık Profili (NSP) ile yaşam kalitesi ve goniometre ile servikal lateral fleksiyon ve rotasyon dereceleri ile değerlendirildi. Gruplar arasında Vizüel Analog Skala (VAS), algometrik ölçümler, palpabl kas spazm derecesi (PKSD), NSP alt grupları ve servikal lateral fleksiyon ve rotasyon derecelerinde istatistiksel olarak anlamlı bir farklılık yoktu. Tedavi sonrası değerlendirmelerde 1. hafta ve 1. ay VAS skorları, algometrik ölçümlerin yüksekliği ve NSP alt gruplarından duygusal reaksiyonlarda iyileşme açılarından kuru iğneleme grubu lehine istatistiksel olarak anlamlı farklılık saptandı. Sonuç olarak TENS ve kuru iğneleme tedavileri ağrının azalmasında etkindir. Kuru iğneleme tedavisi ağrının azalması, algometrik ölçümler ve NSP alt gruplarından duygusal reaksiyonların iyileşmesinde konvansiyonel TENS'e göre daha etkilidir.
This study aimed to compare effectiveness DRY needling with conventional Transcutaneous Electrical Nerve Stimulation (TENS) treatments of myofascial pain syndrome. This study comprised 50 patients who had at last for 3 month trigger point in their upper trapezius muscle with ages between 18-60 years and diagnosed with myofascial pain syndrome (MPS). The patients were randomized by method of choosing numbered and closed envelopes. And divided in two groups which consist of 25 patients. İn grup 1, the patients were treated 5 day a week for 2 week with conventional TENS (50-100 Hz, 100 μsn). İn grup 2, the patients were treated once a week for two week with dry needling to upper trazius muscle (unilateral 2 total 4 trigger point). Stretching exercises for upper trapezius muscle were taught to each group and they were asked to exercise at home. The patients were evaluated with Visual analog scale (VAS) for pain, pain pressure threshold (PPT) measured with an algometer, palpable muscle spasm degree, Nothinghan Health Profile quality of life measurements (NHP) and cervical lateral flexion and rotation degrees measured with a goniometer. There is no statistical significant difference between groups, in VAS, pain pressure threshold, palpable muscle spasm degree, all NHP subgroups scores and cervical lateral flexion and rotation degrees at pre treatment. Post treatment evaluations demonstrated a significant effective in dry needling group that decrease in 1 week and 1 month after treatment VAS scores, increase algometric measurements and decrease emotional reaction in one of subgroup of NHP than conventional TENS group. Both of dry neddling and conventional TENS treatments are effective relieving the pain. Dry needling is much more effective than conventional TENS compare with relieving pain, increases of algometric measurements and decreases emotional reaction score which one of subgroup NHP.
