Normal varyasyon sınırları içinde beyin asimetrisinin çeşitli yaş gruplarında C.T. yöntemiyle değerlendirilmesi

Abstract

Çalışma, Bursa Bölgesinden Uludağ Üniversitesi Tıp Fakültesi Radyoloji Anabilim Dalına başvuran 21 kişinin komputerize beyin tomografileri üzerinde yapıldı. Olguların 119'u kadın ve 95'i erkekti. Yaşları 7 ay ile 67 yıl arasında değişti. Bölgemizde, beyin yarıküreleri arasındaki asimetrilerin dağılımı ve oranları, çeşitli amaçlarla kliniklerimize gelen ve komputerize beyin tomografileri nörolojik ve psikiyatrik hastalıklar yönünden radyolojik olarak normal kabul edilen kişiler üzerinde saptandı. Bu asimetrilerin yaş ile ilişkisi incelendi. Bu amaçla sıfır yaştan başlamak üzere her biri 10 yılı içeren 7 yaş grubu oluşturuldu. Radyogramlar üzerinde cornu anterius'ların maksimum genişliği (FBE), hemisferlerin uzunluğu, septum-caudat genişlik (max SC), lobus frontalis ve occipitalis genişliği, sulcus Sylvil genişliği, hemisfer orta parça genişliği, 3.ventricul ve sulcus Sylvil arasındaki uzaklık (3 V/-S5), ramus posterler sul ci Sylvil ile arkada kafatası iç yüzü arasındaki uzaklık, cornu anteriusların minimum genişliği ve cornu anterius ' ların orta çizgiye maksimum uzaklığı sağ ve sol taraflarda ölçüldü. Ölçümler sonunda şu sonuçlar elde edildi:. Cornu anterius'un maksimum genişliği 11-20 yaş grubu dışındaki tüm yaş gruplarında sol tarafta daha büyüktü ve bu farklılık 31-4O ve 61-70 yaş gruplarında istatistiki olarak anlamlıydı (p CD- 05). Hemisfer uzunluğu 11-20, 41-50 ve 51-60 yaş grupları dışındaki tüm yaş gruplarında sol tarafta daha büyüktü ancak, bu farklılık istatistiki yönden anlamlı değildi.- Septum-caudat genişlik 21-30 yaş grubu hariç 40 yaşına kadar sağ tarafta 40 yaşından sonra sal tarafta daha büyüktü ancak, bu farklılık istatistiki alarak anlamlı değildi. - 21-30 ve 51-60 yaş grupları dışındaki tüm yaş gruplarında sağ labus frontalis daha genişti ve bu farklılık 11-20, 31-40 ve 41-50 yaş gruplarında istatistiki olarak anlamlıydı (p£0.01). - Tüm yaş gruplarında sağ lobus occipitalis daha genişti ve bu farklılık 11-20, 21-30 ve 31-40 yaş gruplarında istatistiki ola rak anlamlıydı (p^0.Ol). - 11-20, 21-30 ve 31-60 yaş grupları dışındaki tüm yaş grupların da sol sulcus Sylvil daha genişti ancak, bu farklılık istatistiki olarak anlamlı değildi. - Tüm yaş gruplarında sağ hemisfer orta parçası daha genişti ve bu farklılık 31-40 ve 51-60 yaş gruplarında istatistiki olarak anlamlı bulundu (p^O.Ol). - 41-50 yaş grubu dışındaki tüm yaş gruplarında sağ tarafta 3U-SS arasındaki uzaklık saldan daha büyüktü ve bu farklılık 31-40 yaş grubunda istatistiki olarak anlamlıydı (p^.0.01). - 41-50 ve 61-70 yaş grupları dışındaki tüm yaş gruplarında ramus posterior sulci Sylvil ile arkada kafatası iç yüzü arasındaki düşey uzaklık sağ tarafta daha büyüktü ve bu farklılık 0-1G, 11-20 ve 31-40 yaş gruplarında istatistiki yönden anlamlıydı (p<0.01). - Sağ cornu anterius'un minimum genişliği her yaşta sağ tarafta daha büyüktü ve bu farklılık 31-40 yaş grubunda istatistiki olarak anlamlıydı (p^O.0l). - Cornu anterius'un orta çizgiye maksimum uzaklığı 50 yaşına kadar sağ tarafta ve 50 yaşından sonra ise sal tarafta daha büyüktü. Bu Farklılık 41-50 yaş grubunda istatistiki alarak anlamlıydı (p^D.C5). Ayrıca. 31-40 ve 61-70 yaş grupları dışındaki tüm yaş gruplarında daha çak sayıdaki kişide arta çizginin sala kaydığı ve her yaş grubunda sağ frontal petalia ve sal aksipital petalia'ya daha çak rastlandığı görüldü. Araştırmamız sonunda asimetri tayininde 1) Bağ labus occi pitalis genişliği, 2) Sağ hemisfer orta parça genişliği, 3) Sağ cornu anterius'un minimum genişliğinin tüm yaş gruplarında ölçülmesi gerektiği ortaya çıktı. Ayrıca kişinin yaş grubu dikkate alınarak a yaş grubu için özellik gösteren diğer ölçümlerin bilinmesinin yararlı alacağı saptandı.
This study was made an cranial computerize tomography of 214 persons from Bursa and applied to Department of Radiology, Medical Faculty of Uludağ Univercity. 119 of cases are women and 95 of them men aged between 7 months to 67 years. In our area, the distributions and ratios of asymmetries between cerebral hemispheres were determined an persons who came to hospital with different aims and whose computerized to mographic scans were accepted radiologically normal from the point of neurologic and psychiatric diseases. In order to show age relations of asymmetries 7 age groups of 10 years were for med. On computerize tomographic scans, the following- parameters at the left and right side were measured: - The maximum width of the frontal horn of the lateral ventric le (FBE) - The anteroposterior length of cerebral hemisphere - The distance from the point where the callosal body meets the septum pellucidum to the nearest point of the head of the caudate nucleus (max SC) - The width of the frontal and the occipital lobes - The width Df the Sylvian fissure - The width of the midportion Df the cerebral hemisphere- The distance between the lateral wall of the third ventricle and the posterior limit of the Sylvian fissure (3U-SS) - The distance between the posterior limit of the Sylvian fissure and the inner table of the occipital bone in the posterior - The minimum width of the frontal horn of the lateral ventricle (the distance between the most medial line of the caudate nuc leus and the midsagittal line) - The distance between the most lateral point of the frontal horn and the midsagittal line. At the end of the study, the following findings were found: - The maximum width of the left frontal horn was larger in all ages except 11-2D years age group. There were significant dif ferences at 5% level in 31-40 and 61-70 years age groups. - The anteroposterior length of the left cerebral hemisphere was larger in all ages except 11-20, 41-50 and 51-60 years age gro ups, but there was not any significant difference. - The max SG was larger at right side until 40 years of age except 21-30 age group. After 40 years left side became more larger than right, but there was not any significant difference. - The width of the frontal lobe was larger at right side in all ages except 21-30 and 51-60 years age groups. There were signi ficant differences at 1% level in 11-20, 31-40 and 41-50 years age groups.' - The width of the right occipital lobe in all age groups was larger and it was significant at 1% level in 11-20,21-30 and 31-40 years age groups.- The widht of the left sylvian fissure was more larger in all ages except 11-2D, 21-30 and 51-60 years age groups. There was not any significant difference. - The width of the midpnrtian of the cerebral hemisphere in all age groups was larger and it was significant at 1 % level in 31-40 and 51-60 years age groups. - The width of the 3U-SS is larger in right side in all ages ex cept 41-50 years age group. It was significant at 1 % level in 31-40 and 51-60 years age graups. - The distance between the posterior limit of the sylvian fissure and the inner table of the occipital bone was larger in right side in all ages except 41-50 and 61-70 years age groups. There was significant differences at 1 % level in 0-10,11-20 and 31-40 years age groups. - The minimum width of the frontal horn was larger in right side in all age groups and it was significant at 1% level in 31-40 years age group. - The distance between the most lateral point of the frontal horn and the midsagittal line was larger at right side until 50 ye ars of age. After 50 years it became larger at left side. The difference was significant at 5 % level in 41-50 years age group. - It was found that in many people the midsagittal line shifted to the- left in all ages except 31-40 and 61-70 years age groups, - It was found that right frontal petalia and left occipital pe- telia were mostly seen in all age groups.Our research in determination of brain asymmetry revealed that the necessity to establish following measurements: 1) The width of the right occipital lobe, 2) The width of the right midportion of the cerebral hemisphere, 3) The minimum width of the right frontal horn of the lateral ventricle in all age groups. It was found that it will be benificial to get other spe cific measurement far those age groups by given special importan ce.
