Abstract

Üçüncü ventrikül tabanını ve komşuluklarını oluşturan anatomik yapılar ventrikülostomi girişimleri yönünden ayrıcalıklı bir öneme sahiptir. Üçüncü ventrikül tabanını oluşturan yapıların anatomisinin ve tabanın büyük kısmını oluşturan hipotalamusun anatomik yapısının bilinmesi yapılacak girişimlerde komplikasyonların önlenilmesi ve ortaya çıkacak bozuklukların anlaşılmasına yardımcı olacaktır. Bu çalışmanın amacı normal ve hidrosefalik beyinlerde üçüncü ventrikül taban bölgesinin histolojik yapısının incelenmesi ve çevre anatomik yapılarla ilişkisinin ortaya konulmasıdır. Çalışmamızda anatomik ölçümler için 23 adet erişkin kadaverik insan beyni kullanıldı. Üçüncü ventrikül tabanını oluşturan yapılar histolojik olarak incelendi ve anatomik ölçümler yapıldı. Üçüncü ventrikül tabanında. Endoskopik üçüncü ventrikülostomide kullanılan balon kateterin ucu ile balon arasındaki mesafe ölçüldü. Ayrıca balon kateterin şişirildiğinde değişen çapı ölçüldü. Kadaverik beyinler üzerinde örnek endoskopik üçüncü ventrikülostomi yapılarak tabanı oluşturan ve komşu olan yapılarla ilişkisine bakıldı. 15 adet normal kraniyal MRI, 15 adet de orta derecede ventriküler genişlemesi bulunan kraniyal MRI görüntüleri üzerinde üçüncü ventrikül tabanının orta noktası ile baziler tepe arasındaki mesafe ölçüldü. Histolojik çalışmada üçüncü ventrikül taban kısmının iç yüzeyini ependim hücrelerinin, dış yüzeyi araknoid membranın, stomayı ise tamamen glial hücrelerin oluşturduğu saptandı. Gliotik yapıdan oluşan bu bölgenin avasküler olduğu izlendi. Normal 15 adet ve ventriküler genişlemesi bulunan 8adet kadaverik beyinler üzerinde yapılan üçüncü ventrikül tabanının ön-arka çapı, transvers çapı, alanı ve tabanın orta noktası ile baziler tepe arasındaki ölçüm ortalamaları bakımından istatistiksel olarak anlamlı bir fark bulundu (p<0,001). Sonuç olarak endoskopik üçüncü ventrikülostomi komplikasyonlarının önlenebilmesi için fenestrasyon mamiller cisimlerin önüne yapılmalı, balon kateter en fazla 0.6 cc serum fizyolojik ile şişirilmeli, preoperatif MRI görüntülemesi mutlaka yapılmalıdır.
The anatomical structures that form the floor of the third ventricle and its surroundings have a distinct importance during ventriculostomy approaches. Detailed knowledge of the anatomy of the hypothalamus which forms most of the floor of the third ventricle aids not only in preventing complications but may facilitate the understating of the resulting deficits as well. The aim of this study was to investigate the histological structure of the floor of the third ventricle and to demonstrate its relation with surrounding anatomical structures in normal and hydrocephalic brains. Twenty three adult human cadaveric brains were used for anatomical measurements in this study. In 8 of the brains there was moderate ventricular enlargement while the remaining 15 brains had normal ventricles. Anatomical measurements were performed at the floor of the third ventricle. The distance between the tip and the baloon of a third ventriculostomy catheter and the change in the diameter of the baloon after inflation was measured. Endoscopic third ventriculostomies were performed on cadaveric brains and the relation between the baloon and related third ventricle floor and surrounding structures were noted. The distance between the basilar apex and center of the third ventricle floor was measured in 15 normal and 15 moderately dilated cranial MR images. Histological analysis showed that the inner surface of the floor of the third ventricle was lined by ependymal cells while the outer surface was formed by the arachnoidal membrane. The stroma was composed of glia which was devoid of vascularity. Distance measurements at the floor of the third ventricle such as the anteroposterior diameter, transverse diameter, the area, the distance between the basilar apex and the center of the third ventricle revealed a statistical difference between the brains with normal and dilated ventricles (p<0.001) In conclusion, in an attempt to avoid the complications of the endoscopic third ventriculostomy the fenestration should be performed in front of the mamillary bodies; the baloon catheter should be inflated to a maximum of 0.6 cc and preoperative MR imaging should be performed.
