Abstract
Spastisite tedavisinde sık kullanılan minimal invazif yöntem olan obturator sinir blokajı yapılması en güç periferik sinir blokları arasındadır. Ultrason son yıllarda sinir blokları için oldukça sık kullanılmaktadır. Çalışmamızın amacı adduktor spastisitesi olan hastalara uygulanan obturator sinir blokajında kullanılan periferik sinir stimülatörü ile ultrason yöntemlerinin karşılaştırılmasıdır. Bizim çalışmamızda adduktor spastisitesi olan 29 hastaya obturator blok uygulandı. Grup I'de (periferik sinir stimülatörü kullanılan) 14 hastaya, grup II'de (ultrason kullanılan ) 15 hastaya obturator blok uygulandı. Fakat iki yöntem arasında etkinlik ve süre açısından fark bulunmadı ve her iki yöntemde de komplikasyon olmadı. Bu konuda daha fazla sayıda randomize kontrollü çalışmaya ihtiyaç vardır.
Obturator neurolytic block is often used to treat patients withadductor spasticity, as a minimal invasive technique and is one of themost difficult peripheral blocks. Ultrasound has gained popularity forperforming nerve blocks in recent years. Our study aims to compareperipheral nerve stimulation and ultrasound guidance used for neurolyticnerve block. Patients were divided into 2 groups: Group I (n=14)patients had obturator block by using nerve stimulator, and group IIpatients (n=15) had the block with ultrasound guidance. No significantdifference was found regarding the efficacy, duration of effect andcomplication rate between the two groups. The performance time tosuccessfully block the nerve was significantly longer in group II (p<0,041). More randomized controlled studies are required on this subject.
