Abstract

Safra yolları kanserleri (SYK) safra kesesi ve safra yollarının epitelinden köken alan invaziv tümörlerden (kolanjiokarsinom) oluşur. SYK'nin genelde ileri evrede görülmesi ve standart kemoterapinin yararının sınırlı kalması, klinisyenleri tedavide yeni yöntemlere yöneltmektedir. Bu çalışmanın amacı IGF-I ve EGF ekspresyonunun SYK'de kemoterapi cevabı ve sağkalım üzerine etkisinin incelenmesiydi. Çalışmaya 32 olgu alındı. Olgulara ait preparatlar EGF ve IGF-I proteini ile immünohistokimyasal olarak boyanmıştır. IGF-I ve EGF ekspresyonu sırasıyla %34 ve %100 olarak saptandı. IGF-I ekspresyonu olan olgularda ortalama yaşam süresi 16.09, olmayan olgularda 11.2 ay idi (p=0,360). EGF aşırı ekspresyonu olan olgularda 12.04 ay, orta/zayıf ekspresyonu olanlarda 13.2 ay idi (p=0.972). IGF-I ve EGF ekspresyonu ile kemoterapi cevabı arasında da istatistiksel olarak anlamlı fark saptanmadı. Sonuç olarak, çalışmamız SYK olan hastalarda IGF-I ve EGF ekspresyonu ile kemoterapi cevabı ve ortalama sağkalım arasında korelasyon olmadığını göstermiştir. Her nekadar prostat, kolon, mide kanserinde olduğu gibi pek çok adenokarsinomda IGF-I ve EGF ekspresyonunun rolü olabilsede SYK'de bu ilişkinin aydınlatılması için daha ileri çalışmalara ihtiyaç vardır.
Bile duct cancer develops from invasive tumor which arises from the epithelium of the of bile tract and gall bladder. Clinicians usually shift their treatments towards novel approaches, due to the were the fact that the limited efficacy of chemotherapy and the occurrence of tumor in advanced stages in many cases with bile duct cancers. The aim of this study was to analyze the effect of IGF-I and EGF on chemotherapy response and survival in bile duct cancers. A total of 32 patients were included into the study. Pathological speasmens were stained with IGF-I and EGF as immunohistochemically. The expression of IGF-I and EGF were found to be 34% and 100%, respectively. The median survival was 16.09 months in the cases having the expression of IGF-I and 11.2 months in those with no IGF-I expression of IGF-I (p=0,360). The median survival was 12.04 months in the cases who had EGF overexpression and 13.2 months in the cases with normal/weak expression of EGF. In our study, no statistically significant difference was found between the expression of IGF-I and EGF and chemotherapy response. As conclusion, our study has showed that there were no correlation between the expression of IGF-I and EGF and the chemotherapy response and survival of patients with bile duct cancer. Although the expression of IGF or EGF may play a role in many adenocarcinomas including prostate, colon and gastric cancer, further researches are required to clarify this role in bile duct cancers.
