Abstract

Vestibüler uyarılmış miyojenik potansiyeller (VEMP-Vestibuler Evoked Myogenic Potentials) yeni bulunmuş olmasına rağmen sakkül fonksiyonunun değerlendirilmesi için klinik kullanıma girmiştir. Ancak, özellikte amplitüdlerde aynı kişinin sağ sol tarafı arasında ve kişiler arasında değişkenlik yüksek seviyededir. Bu sorunun çözümü için EMG kontrolü ile test tekniği ve EMG ölçümü mümkün değilse tansiyon aleti manşonu ile monitorizasyon teknikleri geliştirilmiştir. Ayrıca, her kliniğin kendi normatif değerlerini belirlemeleri gerekmektedir. Bu çalışmanın amacı değişik yaş gruplarında VEMP testinin amplitüd, latans ve eşik değerlerine ait normatif değerleri belirlemek ve tansiyon aleti ile geri besleme tekniğinin, sabit ve kişiye özel basınç değerleri kullanılması ile amplitüdlerin aynı kişinin iki tarafı ve farklı kişiler arasındaki değişkenliğini azaltarak geliştirilmesidir. Çalışma iki aşamalı olarak yapılmıştır. Birinci aşamada tansiyon aleti manşonunda oluşturulan basınçlar ile sternokleidomastoid (SKM) adalede oluşan EMG cevabı arasındaki ilişki araştırılmıştır. İkinci aşamada sabit 40 mmHg basınç (P40) ve SKM adalenin üretebildiği maksimum basıncın %50'si seviyesinde (Pmaks%50) VEMP ölçümleri yapılmıştır. İkinci aşamada yaşları 20 ile 68 arasında değişen 48 erkek 52 kadın toplam 100 gönüllü üzerinde ölçümler yapılmıştır. Kulak bazında cevap oranı %53 seviyesinde gerçekleşmiştir. Geliştirilmiş olan VEMP sandalyesi oturur vaziyetteki kişide tansiyon aleti manşonu kullanarak istenen pozisyonda sadece SKM kas kullanılarak test yapılmasını sağlamıştır. Eşzamanlı EMG yapılamadığı durumlarda tansiyon aleti manşonu kullanımı ile yeterli EMG aktivitesi sağlanabilmektedir. Ancak, kadınlarda yaş ilerledikçe yeterli kas kuvveti sağlanmayabilmektedir. Pmaks%50 ölçümünde amplitüdlerdeki değişkenlik, P40'ta olduğundan daha küçük bulunmuştur. Pmaks%50 kullanımı ile kişiler arası değişkenliğin azaltılması amacına yaklaşılmakla birlikte, yeterli başarı sağlanamamıştır. Amplitüd değişkenliğini ilgilendiren çok sayıda faktörün kontrol altına alınması için daha fazla çalışmaya ihtiyaç vardır.
Although, vestibular evoked myogenic potentials (VEMP) were developed recently, it has been integrated in clinical practice for the evaluation of the saccule function. However, it has high intra-subject and inter-subject variability especially in amplitudes. To solve this problem, EMG controlled test technique and if EMG measurement is not possible, monitorization techniques with blood pressure cuffs were developed. Moreover, each clinic should define its own normative data. The aim of the present study was to define the normative values of VEMP test parameters containing amplitude, latency and threshold, and to improve the feedback with blood pressure cuff technique by decreasing intra-subject and inter-subject variability using constant and individual pressure levels. The study was conducted in two stages. In the first stage, the relationship between the pressure level generated in the blood pressure cuff and EMG response in the sternocleidomastoid (SCM) muscle was investigated. In the second stage, VEMP measurements were performed by using pressure levels at constant 40 mmHg (P40) and 50% of the maximum pressure (Pmax%50) that could be generated by SCM muscle. In the second stage, measurements were performed on 100 volunteers consisting of 48 males and 52 females whose ages ranged between 20 and 68 years. Response rate was 53% on ear basis. Custom-built VEMP chair facilitated the performance of test using blood pressure cuff in a sitting patient with the desired neck position to use only SCM muscle. When simultaneous EMG is not available, use of blood pressure cuff was capable of providing sufficient EMG activity. However, in aged women sufficient muscle strength may not be generated. The variability of amplitudes in Pmax%50 measurements was lesser than that in P40. The use of Pmax%50 come toward the aim of decreasing intra- and inter-subject variability but without sufficient success. Further studies are needed to control many factors that are related to amplitude variability.
