Abstract

Osteogenezis imperfekta (Oİ) tekrarlayan kırıklar nedeniyle yaşam kalitesini bozan kalıtsal bir hastalıktır. Osteogenesis imperfekta, en sık kullanılan sınıflamaya göre dört klinik tipe ayrılmaktadır. Tip I, normal veya hafif kısa boylu, mavi skleralı olup kemik deformiteleri yoktur. Tip II, perinatal dönemde letal seyreden, doğumda birçok kaburga ve uzun kemik kırığı olan, koyu skleralı hastalardır. Tip III, üçgen yüzlü, belirgin boy kısalığı, skolyozu ve intrauterin fraktürlerden dolayı deformiteleri olan gri skleralı bebeklerdir. Tip IV, hafif-orta derecede boy kısalığı, skolyozu ve kemik deformiteleri olan gri veya beyaz skleralı hastalardır. Son yıllarda eski sınıflamaya üç klinik tip daha eklenmiştir. Çalışmamızın amacı Oİ tanısı almış hastalarının geriye dönük değerlendirilip, Oİ'de yeni belirlenebilecek tanı ve tedavi protokollerine yardımcı olabilecek veriler sağlamaktır. Uludağ Üniversitesi Tıp Fakültesi, Çocuk Endokrinoloji polikliniğinde Ocak 2010-Aralık 2014 tarihleri arasında osteogenezis imperfekta tanısı ile izlenen ve yeni tanı alan hastalar çalışmaya alındı. Toplam otuz hasta incelendi, iki hasta takiplerinin devam etmemesi nedeniyle çalışma dışı bırakıldı, 28 hasta çalışmaya dahil edildi. Olgular Sillence sınıflamasına göre otozomal dominant geçen 4 klinik tipe ayrıldı. Çalışmaya alınan yirmi sekiz olgumuzun 14'ü erkek (%50,0), 14'ü kız (%50,0), ortalama yaş 7,48±5,09 yıl idi. Olguların ortalama tanı yaşı ise 25,59±39,59 ay idi. On olguda (%47,6) ailesinde başka bireylerde de osteogenezis imperfekta tanı öyküsü, 7 olguda (%25,0) ise akraba evliliği öyküsü vardı. Olgular Sillence sınıflamasına göre otozomal dominant geçen 4 klinik tipe ayrıldığında; 13 olgu (%46,4) Tip I, 10 olgu (%35,7) Tip III, 5 olgu (%17,9) Tip IV olarak sınıflandırıldı. Olguların tedavi öncesi ortalama bazal DEXA (Dual-Energy X-Ray Absorptiometry) Z skoru -4,76±1,56 iken, tedavi sonrasında -2,27±1,36 (p<0,001) idi. Olguların tedavi sonrası ortalama boy SDS (Standard Deviation Score)'i -2,61±2,78'den; -1,87±2,34'e (p<0,001), ortalama ağırlık SDS'i ise -1,68± 2,71'den; -0,79±2,17'ye (p<0,001), vücut kitle indeksi SDS'i -2,18±2,60'dan; -1,28±2,36'ya (p<0,001) yükseldi. Oİ'de pamidronat tedavisi ile kemik mineral yoğunluğunda artış, kırık sayısında, ağrıda azalma ve mobilitede artış sağlanması ile hastaların yaşam kalitesinde ciddi iyileşmeler sağlanabildiği görülmüştür. Daha etkin bir tedavi bulununcaya kadarpamidronat Oİ tedavisinde en etkin seçeneklerden biri olarak görülmektedir.Ancak pamidronatın kemik üzerinde kronik yan etkileri tam anlamıyla net olarak bilinmediği için bifosfonat kullanımında optimal süre, kullanım aralığı ve dozu konusunda randomize kontrollu daha bir çok çalışmaya ihtiyaç olduğunu düşünmekteyiz.
Retrospective Evaluation of Patients Diagnosed with Osteogenesis Imperfecta Osteogenesis imperfecta (OI) is a hereditary disease that destroys the quality of life by continous bone fractures. Osteogenesis imperfecta according to the most used classification is divided into four clinic types. Type I patientes are normal or slightly short with blue sclera but no bone deformities. Patients with type II show lethal progression in perinatal period, several vertebra and long bone fractures at birth and dark scleras. Type III babies have dark scleras with triangular face, evident short height, scoliosis and deformities due to intrauterine fractures. Type IV are patients with grey or white scleras and slightly short height, scoliosis and bone deformities. In recent years three more clinical types have been added to the previous classification. The objective of our study is to retrospectively evaluate patients diagnosed with OI and come up with helpful data that will assist developing new diagnosis and treatment protocols. Patients followed-up and diagnosed with osteogenesis imperfecta at Uludağ University Faculty of Medicine Pediatric Endocrinology outpatient clinic between January 2010 and December 2014 were included in to the study. A total of thirty patients were investigated, two patients were left out of the study because they did not continue with the follow-up, 28 patients were included in the study. Cases were divided into autosomal dominant four clinical types according to Silence classification. Out of the 28 cases in our study, 14 (50%) were boys, 14 (50%) were girls, and median age was 7.48±5.09 years. Median age at diagnosis was 25.59±39.59 months. Ten cases (47.6%) had a family history of osteogenesis imperfecta, and 7 cases (25.0%) had consanguineous family marriage history. The cases were divided into autosomal dominant four clinic types according to Silence classification as follows; 13 cases (46.4%) type I, 10 cases (35.7%) type III, and 5 cases (17.9%) type IV. The mean average basal DEXA (Dual-Energy X-Ray Absorptiometry) Z score before treatment was -4.76±1.56 whereas after treatment the score was -2.27±1.36 (p<0.001). Median height SDS increased from -2.61±2.78 to -1.87±2.34 (p<0.001), median weight SDS (Standard Deviation Score) increased from -1.68±2.71 to -0.79±2.17 (p<0.001), and body mass index SDS increased from -2.18±2.60 to -1.28±2.36 (p<0.001). Treatment of OI with pamidronate was seen to facilitate increase in bone mineral density, decrease in number of fractures and pain, and improvement of the patient's quality of life by inreased mobility. Pamidronate is one of the most effective treatment of OI until a more effective treatment is found. However since the long-term side effect of pamidronate on bones is not clearly known, we think that randomised controlled studies stil need to be done to determine the optimal period interval, and dose for bisphosphonate use.
