Liposarkomlarda WHO Sınıflandırması ile p53 ve MDM2 ekspresyonlarının immünohistokimyasal olarak belirlenmesi

Abstract

Çalışmanın amacı; liposarkom tanısı almış olguların WHO klasifikasyonuna göre histopatolojik tiplendirmesinin yapılması, p53 ve MDM2 ekspresyonlarının liposarkom tip, derece ve lokalizasyonlarına göre farklarının incelenerek nüks ve tümör çapıyla olan ilişkilerinin belirlenmesidir.Çalışmaya alınan 48 olgunun Hematoksilen-Eozin boyalı kesitleri WHO liposarkom klasifikasyonuna göre değerlendirildi. Olguların parafin bloklarından yapılan kesitlere immünohistokimyasal yöntemle p53 ve MDM2 boyaması uygulandı. Tümör hücrelerinde %10'un üzerinde nükleer boyanma yeterli kabul edildi. İstatistiksel olarak Pearson Ki-Kare testi kullanıldı.Olguların 20'si İDL, 16'sı ML, 7'si PL, 5'i DL olup histopatolojik tiplendirme WHO verilerine uygun olarak bulundu. p53 ve MDM2 immünreaktivitesi açısından tüm olgular birlikte değerlendirildiğinde 48 olgunun 22'sinde p53, 22'sinde MDM2 pozitifliği saptanırken 16 olguda p53 ve MDM2'nin birlikte pozitifliği saptandı. Tüm olgularda p53 ve MDM2 pozitivitesi açısından anlamlı ilişki bulundu. Artan tümör derecesi ile birlikte p53 ve MDM2 pozitivitesinin doğru orantılı olarak arttığı görüldü.Sonuçta; p53 ve MDM2 immünreaktivitesinin liposarkomlarda tanısal açıdan yararlı olabileceğini saptadık. Yüksek dereceli liposarkomlarda p53 ve MDM2 pozitivitesinin arttığını gözlemledik. p53 ve MDM2 ekspresyonunun liposarkom patogenezinde yer aldığı görüşünü destekleyen bulgular elde etmekle birlikte, çalışmamızda olgu sayısının azlığı nedeniyle kesin çıkarımlarda bulunmanın çok sağlıklı olmayacağı kanısına vardık.
The aim of this study was to make the histopathologic typing of cases diagnosed as liposarcoma according to WHO classification, to examine different expression of p53 and MDM2 according to type, grade and localisations of liposarcomas and to determine their relation with recurrences and diameters of tumors.Hematoxylin-Eosin stained sections of 48 cases taken into study were evaluated according to WHO liposarcoma classification. Histologic sections of paraffin blocks were immunostained with p53 and MDM2 antibody. Individual tumor cells were considered positive if more than 10% of the cells displayed nuclear immunostaining. Pearson Chi-Square test was used in statistical evaluation.20 cases were IDL, 16 cases were ML, 7 cases were PL and 5 cases were DL. The histopathologic typing was concommitant with WHO datas. When all the cases were evaluated according to p53 and MDM2 immunostaining. 22 cases were positive with p53, 22 cases were positive with MDM2 and 16 cases were positive with p53 and MDM2. There was meaningful relation with p53 and MDM2 in all cases. With increasing tumor grade positivity p53 and MDM2 were increased in direct proportion.In conclusion; we determined that immunoreactivity of p53 and MDM2 can be useful in diagnosing liposarcomas. We observed the increasing positivity of p53 and MDM2 in high grade liposarcomas. We obtained findings which support the role of p53 and MDM2 expression in liposarcoma pathogenesis but because of minority of cases we decided that it was not true to make find conclusions.
