Abstract

Çalışmamızın amacı, klinik olarak N0 oral kavite kanserli olgularda, onkolojik prensiplerden ödün vermeden gereksiz boyun diseksiyonlarının azaltılması amacına yönelik sentinel lenf nodu biyopsisinin değerini araştırmaktır.Çalışmamız Uludağ Üniversitesi Tıp Fakültesi Kulak Burun Boğaz Anabilim Dalı'nda Mayıs 2006-Mayıs 2008 tarihleri arasında ameliyat edilen klinik olarak N0, oral kavite kanserli, dokuz hastayla prospektif olarak gerçekleştirildi. Hastaların, altısı kadın, üçü erkekti. Bunların sekizi dil gövdesi, biri alt dudak kanseriydi. Tümör evreleri dördü T1, dördü T2 ve biri T4a'dı. Sentinel nodların tespiti için hastalara Nükleer Tıp Anabilin Dalı'nda lenfosintigrafi yapıldı. Takiben 8-16 saat içerisinde ameliyata alındılar. Önce primer tümöral doku eksizyonu yapıldı. Boyun cildi açıldıktan sonra gama prob ile saptanan sentinel nodlar eksize edildi. Bu nodlar donuk kesitte incelendi. Boyun diseksiyonu planlandığı şekilde yapıldı. Sentinel nodlar ve boyun diseksiyonu materyalleri kalıcı histopatolojik incelemeye gönderildi. Sentinel nodların donuk kesit ve kalıcı histopatolojik tanıları ile boyun diseksiyonu materyallerinin histopatolojik tanıları karşılaştırıldı.Hastalarımızın birinde 1, altısında 2, ikisinde 3 adet olmak üzere tamamında sentinel nodlar başarıyla bulunup çıkarıldı. Nodların hepsi ipsilateral boyundaydı. Bunların 2'si seviye 1a'da, 2'si seviye 1b'de, 10'u seviye 2a'da, 4'ü seviye 3'te ve 1'i de seviye 4'teydi. Donuk kesitte nodlardan biri hariç, diğerleri benigndi. Bir hastada sentinel nod benign olmasına rağmen, kalıcı histopatolojik inceleme sonucu, patolojik tanı N1 olarak bulundu.Sonuç olarak, T1 ve T2 evreli sekiz hastamızda sentinel nod sonuçları başarılı olarak bulunmuştur. Diseksiyon materyalleri ile kıyasladığımızda doğru ve güvenilir sonuçlara ulaşılmıştır; fakat T4 evreli hastamızda yanlış negatif sonuç elde edilmiştir. Bu bulgularla, erken evreli oral kavite kanserlerinde bu yöntemin uygulanabileceğini düşünmekteyiz.
The aim of this study is to asses the value of sentinel lymph node biopsy in patients who had oral cavity cancer with clinically N0 neck, in order to decrease unnecessary neck dissections without compromising oncological principles.The study was performed prospectively in nine patients who were operated in Uludag University School of Medicine, Department of Otorhinolaryngology, between May 2006-May 2008 with the diagnosis of oral cavity cancer with clinically N0 neck. Six of the patients were female and three of them were male. Eight of the patients had tongue body cancer and one of the patients had lower lip cancer. Tumor sizes were T1 in four patients, T2 in four patients and T4a in one patient. To demonstrate the sentinel nodes, lymphoscintigraphy was performed in all patients preoperatively, in the Department of Nuclear Medicine. Patients were operated in the next 8-16 hours. Primary tumor was excised first. Then, cervical skin incision for neck disection was done and the sentinel nodes which were identified by the hand-held gamma probe were exicised. These nodes were examined by frozen section. Neck dissections were performed as planned. Sentinel nodes and neck dissection materials were examined in definitive histopathological examination. The results of frozen sectional and definitive histopathological examinations of the sentinel nodes were compared with each other and with the definitive histopatological examination of the each dissection materials.The sentinel nodes were successfully localized and removed in all of the patients. One of the patients had one, six of patients had two and two of patients had three sentinel nodes. All sentinel nodes were in the ipsilateral neck. Two of the sentinel nodes were in the level 1a, two of them were in the level 1b, ten of them were in the level 2a, four of them were in the level 3 and one of them was in the level 4. One of the sentinel nodes was metastatic and the others were benign in the frozen section. One patient was found to be pathologic N1, although the sentinel nodes of this patient were benign.In conclusion, sentinel lymph node biopsy was found to be successful in eight patients with T1 and T2 tumors. The results were accurate and credible when compared to definitve results of the neck dissection materials; but the sentinel lymph node biopsy gave false negative result in one patient who had T4 tumor. According to these results, this method may be feasible for early stage tumors of oral cavity.
