Abstract

Hepatit C virüsü (HCV) tüm dünyada yaklaşık 170 milyon insanı enfekte eden ve %70-80 oranında kronik enfeksiyona sebep olarak siroz ve hepatosellüler karsinoma ilerleyen bir hepatit etkenidir. Günümüzde HCV'ye yönelik etkin bir aşı henüz geliştirilmemiştir. Bu sebeple kronik HCV enfeksiyonunun etkin ilaçlarla tedavi edilmesi gerekmektedir. Genotip 1 HCV ile enfekte hastalarda standart pegile-interferon alfa ve ribavirin içeren ikili tedavi rejimi ile hastaların ancak %40-50'sinde kalıcı viral yanıt (KVY) elde edilmektedir. İnterferon alfa tedavisine yanıtı etkileyen ve konak genomunda 19. kromozomda bulunan IL28B geni ve bu gen yakınındaki single nucleotide polymorphism (SNP)'lerin kalıcı viral yanıt üzerine etkileri daha önce bazı çalışmalarda (Genome-wide Association Studies, GWAS) gösterilmiştir. Bu çalışmada bölgemizde standart ikili tedavi alan hastalarda IL28B rs12979860 ve rs8099917 polimorfizmleri ve KVY üzerine etkileri araştırıldı. Çalışmaya kronik hepatit C (KHC) enfeksiyonu olan, Kafkas kökenli 93 hasta alındı. Median yaş 51 idi (18-70 yaş aralığı). Hastaların %60,2'si kadındı. Hastaların %44,1'inde kalıcı viral yanıt elde edildi. HCV genotip 1 ile enfekte 65 hastada KVY oranı %32,3 idi. HCV genotip 3 ile enfekte 5 hastada KVY oranı %80, HCV genotip 4 ile enfekte 6 hastada KVY oranı %33,3 idi. Genotip 1 ile enfekte hastalarda rs12979860 CC genotipi %27,7 oranında, CT %50,8 oranında, TT %21,5 oranında görüldü. Aynı grupta rs8099917 TT genotipi %47,7; TG %43,1 oranında, GG %9,2 oranında görüldü. Sonuçlarımız ülkemizdeki diğer çalışmalar ile benzer bulundu. Genotip 3 ile enfekte grupta rs12979860 CC genotipi %60, rs8099917 TT genotipi %80 oranında görüldü. Genotip 4 ile enfekte grupta rs12979860 CC genotipi %50, rs8099917 TT genotipi %50 oranında görüldü. Çalışmamızda genotip 1 HCV ile enfekte hastalada rs12979860 CC genotipinde KVY oranı %33,3; C/T genotipinde KVY oranı %24,2; TT genotipinde KVY oranı %50 saptandı. Aynı grupta rs8099917 TT genotipinde KVY oranı %35,5; T/G genotipinde KVY oranı %25, GG genotipinde KVY oranı %50 saptandı. Çalışmamızda genotip 1, 3 ve 4 ile enfekte hastalarda IL28B rs12979860 ve rs809917 varyantları ile KVY arasında ilişki bulunmadı. Çalışmamızda genotip 1 HCV ile enfekte hastalarda erken viral yanıt ile KVY ilişkili bulundu (p<0,001), tedavinin 24. haftasında HCV-RNA pozitifliği ile KVY elde edilmemesi ilişkili bulundu (p=0,013). KVY elde edilmeyenlerde tedavi süresinin daha kısa, lökosit değerlerinin daha düşük olduğu görüldü (p=0,043, p=0,046) Tüm hastalarda ise (genotip ayrımı yapılmadan) erken viral yanıt, 6. ayda HCV-RNA pozitifliği, trombosit, lökosit değerleri ve tedavi süresi ile KVY ilişkili bulundu (p<0,001; p=0,001; p=0,007; p=0,005; p=0,022). Sonuç olarak en azından bölgemiz için KHC tedavisi öncesi IL28B genotipini belirlemenin faydalı olmadığı düşünüldü. Tedavi takibinde EVY ve 6. ayda HCV-RNA değerininin KVY'ı öngörmede belirleyici olduğu sonucuna varıldı.
Hepatitis C virus (HCV) infects about 170 million people worldwide and causes 70-80% chronic hepatitis that progress to cirrhosis and hepatocellular carcinoma (HCC). Vaccine is not available for HCV yet. To prevent progression of infection all HCV infected individuals should be treated. With the combination therapy of pegylated interferon alfa (Peg-IFNα) and ribavirin (RBV), 40-50% of patients infected with genotip 1 HCV achieve a sustained viral response (SVR). Recently several genome-wide association studies (GWAS) have revelaed that single nucleotide polymorphisms (SNP) near or within IL28B (19q13), encoding λ-3, predict spontaneous and treatment induced clearence of HCV. The present study was designed to determine the assosiation between IL28B related SNPs (rs12979860 and rs8099917) and SVR in patients treated with standart dual therapy in our hospital. The number of the patients was 93, all were Caucasians. Median age was 51 years (age range 18-70 years) and 60,2% patients were female. SVR rate was 44,1% in all patients. SVR rate was 32,3% in genotype 1 patients (n= 65) , 80% in genotype 3 patients( n=5) and 33,3% in genotype 4 patients (n=6). In genotype 1 group 27,7% of patients were carrying rs12979860 CC variant, 50,8% were carrying the CT variant and 21,5% were carrying the TT variant. In the same group 47,7% were carriers of the rs8099917 TT variant, 43,1% of the TG variant and 9,2% of the GG variant. Results were similar with the other studies in our country. In genotype 3 group 60% of patients were carrying rs12979860 CC genotype and 80% were carrying rs8099917 TT genotype. In genotype 4 group 50% of patients were carrying rs12979860 CC genotype and rs8099917 TT genotype. HCV genotype 1 patients, rs12979860 CC carriers had a SVR rate of 33,3%; CT carriers had a SVR rate of 24,2% and TT carriers had a SVR rate of 50%. In the same group 35,5% of carriers of rs8099917 TT achieved SVR , 25% of TG carriers achieved SVR and 50% of GG carriers achieved SVR. In the present study we found no association between SVR and IL28B rs12979860 and rs809917 genotypes in genotype 1, 3 and 4 CHC patients. We found association between early viral response (EVR) and SVR (p<0,001), also found that detectable HCV-RNA on the 24. week of treatment predict failure in SVR (p=0,013). In non-SVR patients treatment duration was shorter and leukocyte counts were lower (p=0,043; p=0,046). In all patients EVR, detectable HCV-RNA on the 24. week of treatment, low platelet and leukocyte counts and shorter treatment duration had association with SVR (p<0,001; p=0,001; p=0,007; p=0,005; p=0,022). In conclusion in our region we don't need do determine IL28B genotype before CHC interferon based treatment. In response guided therapy EVR and detection of HCV-RNA at week 24 ongoing treatment could predict SVR.
