Abstract

Çalışmamızda; lokal ileri evre kolorektal kanserli olgularda radyoterapi (RT) ile eşzamanlı oksaliplatin ve kapesitabin uygulanmasının etkinlik ve tolerabilitesini değerlendirmeyi amaçladık.Kasım 2004-Aralık 2007 tarihleri arasında Uludağ Üniversitesi Tıp Fakültesi Muammer Ağım Radyoterapi Merkezi'ne başvuran ve evre II-IV kolorektal kanser tanısıyla eşzamanlı oksaliplatin ve kapesitabin kemoradyoterapisi (KRT) uygulanan 24 olgu retrospektif olarak değerlendirildi. Sekizi (%33.3) kadın, 16'sı (%66.7) erkek olan olguların ortanca yaşı 59 (sınırlar: 36-78) idi. RT'de primer tümör ve bölgesel lenf nodlarına yönelik 45 Gray (Gy) uygulandıktan sonra, primer tümöre yönelik 9-14.4 Gy'lik boost dozu verilerek toplam 54-59.4 Gy'e ulaşıldı. RT haftada beş gün 1.8 Gy/fx'lik konvansiyonel şema ile uygulandı. Olgulara RT süresince her hafta pazartesi günü 75 mg/m²/gün intravenöz oksaliplatin verilirken, kapesitabini 1650 mg/m²/gün sabah-akşam iki doz halinde, hafta içi hergün oral olarak almaları önerildi.Ortanca izlem süresi 19 ay (sınırlar: 1-42) olup, bu sürede iki olguda (%8.3) lokorejyonel yineleme, beş olguda (%20.8) da mevcut hastalığın progresyonu gözlendi. Toplam yedi olguda (%29.2) uzak metastaz geliştiği saptandı. Beş olgu kansere bağlı metabolik yetmezlikten, bir olgu ise kemoradyoterapiden bir ay sonra intrakranyal hemoraji nedeniyle kaybedildi. Yaşam Süresi Tabloları kullanılarak yapılan istatistiksel analizde ortanca iki yıllık genel sağkalım (GSK) ve hastalıksız sağkalım (HSK) oranları sırasıyla %90 ve %79 bulundu.Tedavi sürecinde görülen akut yüksek gradlı (G3-4) hematolojik toksisite üç (%12.5) olguda ve hematolojik dışı toksisite altı (%25) olguda kaydedildi. Kemoradyoterapiye bağlı ciddi periferik nöropati ve/veya el-ayak sendromu hiçbir olguda görülmez iken, beş olguda (%20.8) semptomatik olmayan hiperglisemi saptandı.Sonuçta, gelişen yüksek gradlı toksisite ile lokorejyonel ve uzak yineleme oranlarının az, sağkalım oranlarının da yüksek olması nedeniyle, izlem süresi ve olgu sayısı kısıtlı olmasına karşın, RT ile eşzamanlı uygulanan oksaliplatin-kapesitabin kombinasyonu etkin ve tolere edilebilir olup lokal ileri evre kolorektal kanserli hastalarda kullanılması önerilebilir.
The aim of the study was to investigate the efficacy and the tolerability of concomitant chemoradiotherapy (CRT) with oxaliplatin plus capecitabine on the patients with locally advanced rectal cancer.Twenty-four patients with stage II-IV colorectal adenocarcinoma who were treated with CRT consisting of oxaliplatin plus capecitabine at the Uludag University Faculty of Medicine Muammer Agim Radiotherapy Center, between October 2004-December 2007, were evaluated retrospectively. The median age of the patients was 59 years (range, 36-78). Eight (33.3%) and 16 (66.7%) of the cases were female and male, respectively. Radiotherapy was exposed 1.8 Gy/fx/5 days per week convansionally. The median irradiation dose given to the primary tumor and regional lymph nodes was 45 Gy, followed by a boost dose of 9-14.4 Gy to the primary tumor, bringing to total doses of 54-59.4 Gy. All patients had received intravenous oxaliplatin 75 mg/m²/week plus oral capecitabine 1650 mg/m²/day (twice a day, five days in a week) during radiotherapy.The median 2-year overall survival (OS), and disease-free survival (DFS) rates, statistically analysed by Life Tables, were 90%, and 79%, respectively. During the median follow-up time of 19 months (range, 1-42), two of the patients (8.3%) had showed locoregional failure, and five (20.8%) had progression at the primary tumor site. Distant metastases occured in seven cases (29.2%). Five cases (20.8%) died due to cancer-related metabolic failure, and a patient died because of intracranial heamorrheage one month after the end of the treatment.High grade (grade 3-4) haemathological toxicity was detected in three (12.5%), and non-haemathological toxicity in six cases (25%), respectively. Secondary to chemoradiotherapy none of the patients observed severe peripheral neuropathy and/or hand and foot syndrom, but five patients (20.8%) showed non-symptomatic hyperglicemia.Although the follow-up time and the number of the cases were not sufficient, the low rates of high grade toxicity, the low rates of locoregional or distant failure and the high survival rates that were observed, made us to conclude that the concomittant chemoradiotherapy with oxaliplatin plus capecitabine may be an effective and tolerable treatment modality in locally advanced colorectal cancer.
