Abstract

Broca alanında lisan yeteneğiyle ilgili yapısal asimetri tartışmalıdır. `Broca alanı olarak kabul edilen pars triangularis ve pars operkularis yapısına katılan sulkuslar, bireyler hatta aynı beynin farklı hemisferleri arasında oldukça yaygın anatomik ve morfolojik varyasyonlar gösterir. Bu çalışmada, pars triangularis ve pars operkularis arasındaki morfolojik asimetriyi oluşturan sulkal yapıların varlığı, devamlılığı ve bağlantıları incelendi. Broca alanı ve sağ hemisferdeki homolog bölgenin yüzey alanı ölçümü yapılarak; cinsiyet farkının, pars triangularis şekli ve diagonal sulkus varlığı gibi anatomik varyasyonların yüzey alanı üzerindeki etkilerini analiz etmek amaçlandı. Çalışmamızda, kadavralardan elde edilen 50 beyin, 100 hemisfer kullanıldı. Beyinler, hazırlık aşamasından sonra cerrahi mikroskop altında incelenerek ilgili anatomik bölge fotoğraflandı. Photoscape 3.5.6 programı kullanılarak ilgili sulkal ve giral yapılar işaretlendi. Çekilen ölçekli fotoğraflarda Adobe Acrobat XI Pro programı kullanılarak yüzey alanı ölçümü yapıldı ve veriler istatistiksel olarak analiz edildi. İnferior frontal girus, ilgili sulkal ve giral yapılarda yaygın bireysel anatomik ve morfolojik varyasyon saptandı. Sol hemisferde diagonal sulkus varlığı anlamlı olarak yüzey alanını arttırdı. Taraf, pars triangularis şekli ve cinsiyet farklılığı yüzey alanına anlamlı oranda etki etmedi. Bu bölge ve komşu yapıların anatomi ve varyasyonlarının bilinmesi güvenli cerrahi için zorunludur. Bu çalışmada sağ ve sol taraf arasında yüzey alanı açısından fark saptanmadı. Diagonal sulkus varlığının yüzey alanını arttırdığı tespit edildi. Ancak, hemisferler arasında diagonal sulkus varlığı açısından anlamlı fark olmadığından; bu bulgu Broca alanı için yapısal asimetriyi desteklememektedir.
The structural asymmetry of Broca's area about language ability is controversial. The sulci that join in the structure of pars triangularis and opercularis, considered as `Broca?s area?, represents wide anatomical and morphological variations between individuals, and even in different hemispheres of the same brain. In this study; the presence, continuity and the connections of the sulcal structures that form the morphological asymmetry of pars triangularis and pars opercularis are investigated. The aim was to analyse the effects of gender difference, anatomic variations such as pars triangularis shape and the presence of the diagonal sulcus on surface area, by measuring Broca's area and the homologous area on the right hemisphere. In our study, we used 50 brains, 100 hemispheres obtained from cadavers. After preperation procedure was completed, the brains were examined using surgical microscope, the relevant anatomical region was photographed. Related sulcal and gyral structures were marked by Photoscape 3.5.6 software. Surface area measurements were done on photos taken with scale by using Adobe Acrobat XI Pro software and the the datas were analysed statistically. Considerable individual anatomical and morphological variations between inferior frontal gyrus, related sulcal and gyral structures was detected. The presence of a diagonal sulcus on the left hemisphere significantly increased the surface area. Side, pars triangularis shape and gender difference did not significantly affect the surface area. Knowledge of anatomy and variations in this region and the neighboring structures are mandatory for safe surgery. In this study, a difference between right and left sides was not determined. It was found that the presence of diagonal sulcus increases the surface area. However, since there is no significant difference between the hemispheres in the presence of diagonal sulcus, this finding does not support the structural asymmetry of Brocas area.
