Although the amount of research in the field of pathogenic fungi has also increased, there is still a need for studies on fungal virulence. In this investigation, we focused on testing bloodstream-identified Candida albicans in experimental in vitro virulence assays. The proteinase enzyme activities of 30 C. albicans strains isolated from the bloodstream and sterile body fluids were investigated (15 colonizations, 15 infections). The spectrophotometric and plate methods were used to determine proteinase activity. The correlations between the 2 methods were compared. Our purpose was to present the diagnostic and therapeutic management and outcome of these patients. STUDY DESIGN: Case series with chart review SETTING: Tertiary care university hospital SUBJECTS AND METHODS: Twenty-six patients, who were operated on because of AIFRS between September 1999 and June 2009. were retrospectively evaluated in this study. Endoscopic surgery was used in 19 patients, and open surgical debridement was performed in seven patients RESULTS: Overall survival rate of the patients in the open surgery group (4 of 7, 57 1%) was similar to that of the endoscopically treated group (9 of 19:47 3%) Thirteen patients (50%) died of complications related to the underlying disease (9 of 13; 69 2%) and AIFRS (4 of 13, 30 7%). Bu raporda, 34 yaşında akut lenfoblastik lösemi tanısı olan bir erkek hastada agresif nörocerrahi, intrakaviter amfoterisin B tedavisi ve vorikonazol kombinasyonu ile başarıyla tedavi olan bir serebral aspergilloz olgusu sunulmuş ve nöroaspergillozun tedavisinde cerrahi tedavi, intrakaviter tedavi ve antifungal tedavinin rolleri tartışılmıştır. Amfoterisin B tedavisi altında dizartri ve sağ hemiparazi gelişen olgumuzda beyin manyetik rezonans görüntülemede sol paryeto-oksipital bölgede 7 cm çapında lezyon saptanmıştır. Tanı amaçlı yapılan cerrahi girişim aşırı kanama nedeniyle başarıyla gerçekleştirilememiştir. Lezyondan alınan aspirat örneğinin kültüründe Aspergillus flavus üremesi saptanmıştır. Tedavi kaspofungin ve vorikonazol kombinasyonu olarak değiştirilmiştir. Tedavi altında enfeksiyonun ilerleme göstermesi üzerine hastaya ikinci cerrahi girişim uygulanmış ve lezyonun boyutunda gerileme tespit edilmiştir. Cerrahi tedaviyi takiben vorikonazol ve kaspofungin kombinasyon tedavisine ek olarak intrakaviter amfoterisin B uygulaması yapılmıştır (0.3 mg/gün 15 gün). Kaspofungin tedavisine 42 gün devam edildikten sonra kesilmiştir. Vorikonazol tedavisine ise 100 gün devam edilmiştir. Bu dönemde beyindeki lezyon tama yakın düzelme göstermiştir. Ancak, hastanın takibi sırasında lösemi relapsı görülmüş ve kemoterapi sırasında aplazik dönemde gelişen nötropenik tiflitis tablosu ile hasta kaybedilmiştir. Serebral aspergillozlu olgularda erken dönemde radikal cerrahi uygulanması sonucunda daha iyi sonuçlar elde edildiği belirtilmektedir. Antifungal tedavide kullanılan ilaçların vorikonazolde olduğu gibi kan-beyin bariyerini geçebilme özelliğine sahip olması gerekmektedir. Tüm lezyonlar düzelinceye ve altta yatan predispozan faktörler geri dönünceye kadar tedaviye devam edilmelidir. Bu çalışma, hematolojik maligniteli hastalarda kandidemi prevalansını, mortalite ile ilişkili kontrol edilebilir risk faktörlerini ve antifungal direnci belirlemeyi, ampirik antifungal tedaviye rehberlik edecek ve enfeksiyon kontrol stratejilerinin geliştirilmesine katkıda bulunacak bilgiler elde etmeyi amaçlamaktadır. Bursa Uludağ Üniversitesi Tıp Fakültesi Hastanesi Hematoloji Kliniğinde 2009-2016 yılları arasında Candida kan kültürü pozitif saptanan hematolojik maligniteli hastaların demografik verileri, laboratuvar sonuçları, antifungal duyarlılıkları ve tedavi sonuçları geriye dönük olarak incelendi. Hematolojik maligniteleri olan 2489 hastanın 45’inde 45 kandidemi atağı saptandı. Bu hastaların büyük çoğunluğu akut lösemi hastalarıydı (%75.5). Çalışmamızda hematolojik malignitesi olan hastalarda kandidemi insidansını %1.8 bulduk; akut lösemili hastalarda ise bu oran %2.3 idi. Hastaların tür dağılımında C. albicans dışı türler hakimdi (38/45, %84,4). Genel olarak yatan hastalara kıyasla hematolojik malignitesi olan hastalarda C. krusei ve C. tropicalis daha yaygındı. C. parapsilosis her iki grupta da kandidemi için yaygın bir etkendi. Antifungal direnç, flukonazole karşı sadece iki C. parapsilosis izolatında antifungal direnç gözlendi. Toplam 30 günlük ölüm oranı %55,5 idi. Mortalite ile ilişkili bağımsız risk faktörleri hipoalbuminemi, aktif hastalık, septik şok ve monoterapi almaktı. Sonuç olarak, tedavisindeki gelişmelere rağmen, kandida ilişkili mortalitede önemli bir azalma sağlanamamıştır. Kandidemi tedavisinde lokal epidemiyolojik çalışmalar hala önemini korumaktadır. C. parapsilosis, kötü kateter yönetiminin bir göstergesi olarak kabul edilir. Çalışmamızda C. parapsilosis yüksek oranda tespit edilmiş ve antifungale direnç gösteren tek tür olmuştur. Bu, kandidemi ile mücadelede iyi kateter yönetiminin öneminin yanı sıra ampirik antifungal tedavi stratejilerinde epidemiyolojik çalışmaların önemini vurgulamaktadır.Item Alt solunum yolu örneklerinden üretilen aspergillus türlerinin in-vitro antifungal duyarlılığının mikrodilüsyon ve e test yöntemleriyle araştırılması(Uludağ Üniversitesi, 2007) Efe, Şirin; Ener, Beyza; Uludağ Üniversitesi/Tıp Fakültesi/Enfeksiyon Hastalıkları Anabilim Dalı.Son yıllarda, antifungal duyarlılık testleri, invaziv mikozların artan insidansı nedeniyle gittikçe daha fazla önem kazanmaya başlamıştır. Bu çalışmada da, 222 Aspergillus suşunun (136 A fumigatus, 56 A flavus, 20 A niger ve 10 A terreus), Clinical and Laboratory Standards Institute (CLSI) referans mikrodilüsyon ve E test yöntemleriyle amfoterisin B, ketokonazol, itrakonazol ve vorikonazol aktivitesi değerlendirildi. Kaspofungin etkinliğine ise sadece E test ile bakıldı. 35°C’de, 24 ve 48 saatlik inkübasyonlar sonunda her iki yöntemle de; MİK aralıkları, MİK50, MİK90 ve geometrik ortalamalar belirlendi. Yöntemler arasındaki % uyum, ±1 ve ±2 dilüsyonlarda hesaplandı. Mikrodilüsyon yönteminde, en etkili ilaç vorikonazol olarak bulundu ve izolatların %90’dan fazlası ≤1 µg/ml MİK değerleriyle inhibe oldu. Bunu itrakonazol, amfoterisin B ve ketakonazol takip etti. E test yönteminde ise kaspofungin MİK değerleri, vorikonazolden daha iyi idi ve tüm suşlar ≤0,5 µg/ml MİK değerleriyle inhibe oldu. Vorikonazol ve kaspofunginin, tüm Aspergillus türlerine etkinliği çok iyi bulundu, fakat amfoterisin B’nin, aktivitesinin, A flavus ve A terreus’da düşük olduğu görüldü. İki yöntem arasındaki % uyumun, ±1 ve ±2 dilüsyonlarda her iki inkübasyon süresinde de, en yüksek vorikonazolde olduğu saptandı (sırasıyla % 83-97 ve %82-96). This study aims to measure the serum levels of interleukin (IL)-23, IL-17, IL-1 beta, tumor necrosis factor (TNF)-alpha, IL-10, transforming growth factor (TGF)-beta, C-reactive protein (CRP), procalcitonin (PCT), and serum amyloid A (SAA) in cases of candidemia and to compare them with those observed in cases of bacteremia. For this purpose, the serum cytokine levels from 50 patients with candidemia were compared with those of 14 patients with polymicrobial sepsis, 30 patients with bacteremia, and 27 healthy control subjects. The cytokine levels were studied using sandwich ELISAs according to the manufacturer protocol. The serum levels of TGF-beta, IL-23, and IL-17 were found to be significantly higher in the candidemia group in comparison with the samples from those with bacteremia and healthy controls. The PCT and SAA levels were higher in samples from the group with bacteremia those from individuals with candidemia and the healthy control group. Assuming an IL-17 level threshold of > 38.79 pg/ml, the sensitivity and specificity were 38% and 96.6%, respectively but considering an IL-23 threshold of > 59.97 pg/ml, the sensitivity and specificity valueswere found to be 72% and 60%, respectively. The sensitivity and the specificity of the TGF-beta levels were found to be 85.71% and 53.33%, respectively, when the TGF-beta threshold is > 560 pg/ml. PCT and SAA demonstrated a superior performance for the differentiation of candidemia and bacteremia. However, aspergillus osteomyelitis in vertebrae is rare. We report a case of aspergillus spondylodiskitis after pulmonary aspergillosis in a renal transplant recipient. He was treated by antifungal therapy and surgical intervention. An international multicenter cohort study (AspICU) established criteria for diagnosis of invasive pulmonary aspergillosis (IPA) in intensive care units. In our study, patients with Aspergillus spp. growth in deep tracheal aspirate (DTA) samples in ICU were evaluated according to AspICU criteria.Materials and Methods: This study is a retrospective study. DTA samples were collected from the Pandemic and Reanimation ICU and performed in the Medical Microbiology Laboratory by separated two periods; pre-pandemic (1 March 2019-31 December 2019) and post-pandemic (1 March 2020-31 December 2020). Cases with Aspergillus spp. growth in the DTA samples in the Pandemic ICU were evaluated as COVID 19 associated pulmonary aspergillosis (CAPA) according to AspICU criteria.Results: While Aspergillus spp. was grown in the DTA of three patients in 2019 and five patients in 2020 in the Reanimation ICU, and 11 patients in the Pandemic ICU. Growths belonging to one patient from both Reanimation (2019) and Pandemic ICUs were considered as colonization. Other growths were interpreted as IPA according to AspICU criteria. When the incidence rates according to 10000 patient days were compared, the incidence rate increased significantly in 2020 (19.1) (p< 0.001) compared to 2019 (3.4); In 2020, it was determined that it increased significantly in the Pandemic ICU (40.4) (p< 0.001) compared to Reanimation ICU (9.2).Conclusion: It should not be forgotten that intensive care patients are also at risk for IPA, especially after viral infections (such as COVID-19, Influenza). Although the incidence of IPA was not very high, it was observed that it tended to increase according to our study. Early diagnosis allowing early treatment may improve the prognosis. However, this goal remains difficult to achieve. When the diagnosis is confirmed, it is often already too late. Despite antifungal treatment, the mortality rate is high. Patients with immunosuppression show a high mortality rate. We present five patients of ages ranging between 34 and 43 years who displayed aspergillus pneumonia between 1991 and 2000. All patients received cyclosporine, azathioprine, and prednisone for maintenance immunosuppressive therapy. Their ages ranged from 34 to 43 years with the onset of infection between 1 to 25 months posttransplant. In all cases, the infection was localized to the lungs. Standard methods of fungal culture and identification were used. No coinfections with tuberculosis or other fungi or bacteria were, identified. Three cases were successfully treated but two patients showed deterioration despite appropriate therapy and died. Among patients with solid organ transplantation, lung, liver, and renal transplant patients are at the highest risk of developing aspergillus lung infections. Generally, Fusarium species are intrinsically resistant to all systemic antifungals. We investigated whether the presence or absence of the ability to produce biofilms across and within Fusarium species complexes is linked to higher resistance against antifungals. A collection of 41 Fusarium strains, obtained from 38 patients with superficial and systemic infections, and three infected crops, were tested, including 25 species within the Fusarium fujikuroi species complex, 14 from the Fusarium solani species complex (FSSC), one Fusarium dimerum species complex, and one Fusarium oxysporum species complex isolate. Of all isolates tested, only seven strains from two species of FSSC, five F. petroliphilum and two F. keratoplasticum strains, recovered from blood, nail scrapings, and nasal biopsy samples, could produce biofilms under the tested conditions. In the liquid culture tested, sessile biofilm-forming Fusarium strains exhibited elevated minimum inhibitory concentrations (MICs) for amphotericin B, voriconazole, and posaconazole, compared to their planktonic counterparts, indicating that the ability to form biofilm may significantly increase resistance. Birçok yeni antifungal ilacın geliştirilmesine rağmen epidemiyolojik çalışmalar Candida izolatlarındaki antifungal direncin ciddi bir problem haline gelmeye başladığını göstermiştir. Antifungal duyarlılığı ölçmek için iki standart yöntem vardır ve bu çalışmada kandan elde edilen Candida izolatlarının amfoterisin B, flukonazol, itrakonazol, vorikonazol, posakonazol ve anidulofungin duyarlılığını belirlemek için “Clinical and Laboratory Standards Institute (CLSI)” ve “European Committee on Antimicrobial Susceptibility Testing (EUCAST)” sıvı mikrodilüsyon yöntemlerinin karşılaştırılması amaçlanmıştır. Çalışmada 74 Candida albicans, 67 Candida parapsilosis, 30 Candida glabrata ve 18 Candida tropicalis izolatı kullanılmıştır. Anidulofungin hariç, minimum inhibitör konsantrasyon değerleri, CLSI yönteminde 24 ve 48 saat inkübasyondan sonra, EUCAST yönteminde sadece 24 saat inkübasyondan sonra belirlenmiştir. Her iki yöntemle elde edilen MİK değerleri ± 2 dilüsyon sınırları içerisinde ise uyumlu olarak kabul edilmiştir. Her antifungal için yöntemler arasındaki kategori uyumunda klinik sınır değerler ve epidemiyolojik eşik değerlerden yararlanılmıştır. Yöntemler arasındaki uyum (± 2 dilüsyonda) tür, ilaç ve inkübasyon zamanına bağımlı olarak bulunmuştur. Yirmi dört saat inkübasyon sonunda amfoterisin B, itrakonazol, posakonazol ve anidulofunginde iyi (> %90) kategorisinde uyum bulunurken, flukonazol ve vorikonazolde, özellikle nispeten daha yavaş üreyen C.glabrata ve C.parapsilosis izolatlarında daha düşük kategorisinde uyum (< %85) bulunmuştur. Mükemmel kategori uyumu (%100) amfoterisin B/C. parapsilosis, C.glabrata, C.tropicalis ve andilofungin/C.albicans, C.glabrata, C.tropicalis ile görülürken en kötü uyum, posakonazol ve C.albicans (24 saate %71.6; 48 saatte %73) arasında saptanmıştır. Tedavide sık kullanılan ilaçlar olan flukonazol ve anidulafungine her iki yöntemle tespit edilen direnç C.albicans (sırasıyla %1.3; %2.7), C.glabrata (sırasıyla 0%, %3.3) ve C.tropicalis’de (sırasıyla 0%, %5.6), in vitro azalmış duyarlılık C.parapsilosis ve flukonazolde (24 saatte %11.9; 48 saatte %17.9) görülmüştür. Flukonazol ve vorikonazol arasında çapraz direnç üç C.parapsilosis izolatında saptanmış ve bir C.albicans izolatında ise çok ilaca direnç (flukonazol, itrakonazol, posakonazol ve anidulafungin) tespit edilmiştir. Çalışma sonucunda, her iki yöntemin benzer olduğu ve laboratuvarların tercihine göre kullanılabileceği gözlenmiştir. CLSI antifungal duyarlılık sonuçlarının 24 saat inkübasyon sonunda değerlendirilebileceğinin ancak bazen, C.glabrata gibi yavaş üreyen türlerde 48 saat inkübasyon sonucunda değerlendirme yapılması gerektiğinin önemli olduğu saptanmıştır.Item Candida türlerinin tanımlanmasında iki farklı isitemin karşılaştırılması(Uludağ Üniversitesi, 2014) Gayıbov, Ülkü; Ener, Beyza; Uludağ Üniversitesi/Tıp Fakültesi/Tıbbi Mikrobiyoloji Anabilim Dalı.Fırsatçı mantarlar, nozokomiyal enfeksiyonların önemli nedenlerinden biri olup, C. albicans ve diğer Candida türleri hastalardan artan sıklıkta izole edilmektedir. Bunlardan bir kısmı yaygın kullandığımız antifungallere dirençli olduklarından, uygun tedaviye başlamak için hızlı ve doğru tanımlama gerekir. Bu çalışmada da API ID 32 C (bioMerieux France) ve Phoenix Yeast ID (Becton, Dickinson Diagnostics, Sparks, MD) tanımlama sistemlerinin karşılaştırılması ve morfolojik bulguların tanımlamaya etkisinin değerlendirilmesi amaçlanmıştır. İzolatlar, çalışma döneminde Uludağ Üniversitesi Hastanesi'nde yatan hastaların örneklerinden elde edilmiştir. Tüm izolatlar çimlenme borusu ve mikroskobik morfoloji açısından incelenmiş ve üretici firmaların önerileri doğrultusunda tanımlama yapılmıştır. İki sistem arasındaki uyum yüzde olarak hesaplanmış ve uyumsuz sonuçlar, morfolojik bulgularla çözümlenmeye çalışılmıştır. Yüz otuz yedi hastadan toplam 211 maya izolasyonu olmuştur. İzolatların 159'unda aynı tanımlama elde edilmiş olup, %75,4 uyum saptanmıştır. Uyum, sık izole edilen türlerde (%81,7), seyrek olanlara (%38,7) göre anlamlı olarak (p=0.034; x2 test) daha yüksek bulunmuştur. Her ne kadar anlamlı olmasa da (p=0.31; x2 test) morfolojik bulguların eklenmesi ile uyum oranı (%88,1) artmıştır. Uyumlu tanımlama bulgularına göre, en sık izole edilen tür C. albicans (%44,1) olmuş, bunu C. tropicalis (%9,9), C. glabrata (%9,5), C. parapsilosis (%8,5) ve C. kefyr (%8,1) izlemiştir. Tür tanımlanmasında moleküler yöntemler daha doğru olsa da, klinik mikrobiyoloji laboratuvarlarında halen fenotipik yöntemlerden yararlanılmaktadır. Bu çalışmada da iki sistemin karşılaştırılabilir olduğu ve morfolojinin tanımlamaya pozitif etkisi görülmüştür. Phoenix Yeast ID Panel kullanımı ve yorumlanması kolay olup, API ID 32 C'den 24 saat daha erken sonuçlanmaktadır. Case report: A. lentulus was isolated as the cause of pneumonia from a patient who had renal transplantation 4 months ago. The patient received immunosuppressive treatment after transplantation. A. lentulus was isolated from his sputum as an agent in pneumonia developed 4 months after the transplantation. Leukocytes, blastospores, and hyphae were seen in both Gram- and Giemsa-stained smears of the sputum. The isolate was identified by using the Maren A. Klich algorithm and molecular methods and confirmed by the reference laboratory of the CBS Fungal Biodiversity Centre ( The Netherlands). In the susceptibility tests of the isolate, minimal inhibitory concentrations for amphotericin B, voriconazole, posaconazole, and caspofungin were found to be 0.5 mu g/mL, 0.25 mu g/mL, 0.125 mu g/mL, and 0.25 mu g/mL, respectively. The patient recovered with voriconazole treatment (2x200 mg/day). Methods: We investigated nasopharyngeal swabs for respiratory viruses between March 2020 and February 2021 during the first year of pandemic in Turkey. We used QIAStat Dx Respiratory panel (Qiagen, Germany) in QIAStat Dx (Qiagen, Germany) for detection of respiratory viruses between March 2020 and February 2021. Respiratory panel kit included influenza A, B, influenza A H1N1, rhinovirus/enterovirus, parainfluenza (PIV) 1,2,3,4, coronaviruses (CoVs) NL 63, 229E, OC43 and HKU1, human metapneumovirus (MPV) A/B, bocavirus, respiratory syncytial virus (RSV) A/B and adenovirus. Results: We retrospectively analyzed the results of 319 nasopharyngeal swab samples. The average age of 199 (62.4%) male and 120 (37.6%) female patients between the ages of 0-92 was 16 years. We found that 101 (31.7%) samples were positive for viruses. Rhino/enteroviruses were the most common viruses in all age groups. Influenza positivity rate during the first year of pandemic declined to 2.3% from 17.3% among the previous year. MPV infection activity did not change during the pandemic. Discussion: According to our findings we argue that epidemiology of respiratory viruses has changed during the pandemic period. Despite the current clinical focus on the COVID-19 pandemic, clinicians should keep in mind that rhino/enterovirus and MPV infections may mimic COVID-19 and respiratory infections should be differentially diagnosed with rapid multiplex kits containing SARS-CoV-2, rhino/enterovirus and MPV. In this study, our aim was to assess the efficacy of tea tree oil in Demodex treatment on caucasian patients in an industrialized region of Turkey, and to develop a systematic scoring system for extremely accurate diagnosis in the absence of advanced facilities. Material/Methods: Charts of 412 patients with blepharitis were reviewed. A group of 39 out of 412 cases were identified as chronic and treatment-refractory, and therefore were enrolled in this study. Eyelashes from each of the lower and upper eyelids of both eyes were evaluated at x40 and x100 magnification using light microscopy. Treatment was started with 4% tea tree oil eyelid gel and 10% eyelash shampoo. Symptoms and findings were scored according to the most common complaints. Results: The mean age of the patients was 54.1 +/- 15.4 years. Seventeen (43.5%) patients were male and 22 (56.5%) patients were female. In 30 out of the 39 patients (76.9%) D. folliculorum was detected. Symptoms disappeared in 25 patients. The mean score of patients who were Demodex-negative was 2.7 +/- 1.0, and the mean score of patients who were Demodex-positive was 3.8 +/- 1.6 (p=0.047). Ninety-four percent of those with a score of 4 and over were found to be Demodex-positive (p=0.025). Conclusions: Treatment with tea tree oil can be successful. Cultures were collected from the different anatomical sites of all the patients within 24 h of diagnosis of candidemia. Molecular similarities between identical species colonised with Candida species were evaluated via karyotyping. The colonisation index, as developed by Pittet et similar to al. was calculated using screening culture results from patients. Among the 40 patients screened for colonisation, 35 (87.5%) had colonisation of at least one anatomical site. Twenty-six (74.3%) of the 35 patients with colonisation in any of the three anatomical sites (respiratory, rectum and urinary sites) were shown to be colonised with the same species that caused candidemia. When the anatomical sites were compared with each other, no significant difference was observed at the species level in terms of colonisation index. The colonisation index (=0.5) positivity rate was 74% in patients with candidemia. This study was designed to investigate the galactomannan assay optical density (OD) indices relative to the culture results in bronchoscopic samples obtained from neutropenic and non-neutropenic patients. Galactomannan OD indices from 1427 samples from 2005 to 2012, which were sent from 839 patients and were composed of bronchial lavage (BL=727) and bronchoalveolar lavage fluids (BAL=700), were retrospectively analysed. The recovery rates of Aspergillus species from these specimens were 9.4% from the combined patient group and 13.3% from the neutropenic group. Aspergillus fumigatus complex was the most frequently isolated species. The mean and median OD indices of the positive and negative culture samples are approximately 5 and 1, respectively, and 91% of all culture-positive samples have 1 OD index value. The receiver-operating characteristics curve analysis demonstrated that the feasibility of the Aspergillus galactomannan assay and Aspergillus galactomannan test has superior accuracy in BAL compared to BL fluids, and the test is not affected by the immune status of the patient. We suggest that the Aspergillus galactomannan test, which uses bronchoscopic material, leads to an earlier diagnosis and if the OD index is found 1, fungal growth can be expected.
